Objective To investigate the value of ultrasonic elastic strain rate ratio(SR)in predicting cervical lymph node metastasis in papillary thyroid carcinoma(PTC).Methods The clinical data of 68 patients(68 lesions)with PTC confirmed by postoperative pathology in our hospital from January 2022 to December 2023 were studied.All patients underwent routine two-di-mensional ultrasonography and ultrasound elastography before surgery.Lymph node metastasis in all PTC patients was recorded,and the values of SR and A/B(B/A)in nodule area were compared between PTC patients with lymph node metastasis and those without,and the diagnostic value of SR,B/A and combined detection in cervical lymph node metastasis of PTC was analyzed.Re-sults A total of 68 patients with lymph node metastasis were included in the metastatic group in 27 cases and in the non-metastatic group in 41 cases.The SR and B/A values in the metastasis group were higher than those in the non-metastasis group,with statistical difference(P<0.05).The combination of SR and B/A values had the highest application value in predicting PTC cervical lymph node metastasis,with AUC of 0.958,sensitivity of 0.926 and specificity of 0.854.The second value was B/A,AUC was 0.854,sensitivity was 0.815,and specificity was 0.805.The AUC of SR is 0.752,the sensitivity is 0.704,and the specificity is 0.610.Conclusion The combination of SR and B/A values has high clinical application value in the prediction of PTC cervical lymph node metastasis,with high diagnostic sensitivity and specificity.It can be used as an effective method in the differential di-agnosis of PTC cervical lymph node metastasis,and is worthy of promotion and application.
