摘要
目的 分析甲钴胺联合布洛芬缓释胶囊治疗腰椎间盘突出症(LDH)对血清疼痛递质及白介素-23(IL-23)、基质金属蛋白酶-9(MMP-9)的影响.方法 回顾性选取 2021 年 1 月至 2024 年 3 月上饶东信第五医院收治的 160 例LDH患者,根据治疗方案的不同,分为对照组(n=74)与联合组(n=86),对照组给予布洛芬缓释胶囊治疗,联合组在对照组基础上联合甲钴胺治疗,疗程均为 4 周.比较两组患者接受治疗前后的总有效率、血清疼痛递质[前列腺素E2(PGE2)、5-羟色胺(5-HT)、神经生长因子(NGF)]、IL-23、MMP-9、视觉模拟评分(VAS)、Oswestry腰椎功能障碍指数(ODI)评分及不良反应发生率.结果 治疗后,联合组总有效率 88.37%(76/86)高于对照组 74.32%(55/74)(P<0.05);两组患者的血清疼痛递质(PGE2、5-HT、NGF)、IL-23 及 MMP-9 均明显降低(P<0.05),联合组均低于对照组(P<0.05);两组患者的 VAS、ODI 评分均明显降低(P<0.05),联合组的VAS、ODI评分均低于对照组(P<0.05);治疗期间,患者的不良反应发生率差异无统计学意义(P>0.05).结论 甲钴胺联合布洛芬缓释胶囊治疗LDH患者疗效良好,能够有效调节血清疼痛递质及IL-23、MMP-9 表达水平,缓解患者腰部疼痛,促进腰椎功能的恢复.
Abstract
Objective To analyze the effect of mecobalamin combined ibuprofen capsule for lumbar disc herniation on serum pain transmitter and interleukin-23(IL-23)and matrix metalloproteinase-9(MMP-9).Methods Retrospectively selected 160 LDH patients admitted to Shangrao Dongxin Fifth Hospital from January 2021 to March 2024 were divided into the control group(n=74)and combination group(n=86)according to the different treatment plans.The control group was given ibuprofen sustained-release capsule,and the combination group was treated with mecobalamin in the control group for 4 weeks.Total response rate,serum pain transmitters[prostaglandin E2(PGE2),5-hydroxytryptamine(5-HT),nerve growth factor(NGF)],IL-23,(MMP-9,visual analogue scale(VAS),Oswestry disability index(ODI)score and incidence of adverse effects were compared.Results After the treatment,The total response rate of the combined group was 88.37%(76/86)higher than that of the control group by 74.32%(55/74)(P<0.05);Serum pain transmitters(PGE2,5-HT,NGF),IL-23,and MMP-9 were significantly decreased in both groups(P<0.05),The com-bined group was lower than the control group(P<0.05);The VAS and ODI score were significantly lower in both groups(P<0.05),The VAS and ODI score of the combined group were lower than the control group(P<0.05);There was no significant difference in the incidence of adverse reactions during the treatment period(P>0.05).Conclusion And ibuprofen sustained-release capsule is good in LDH patients,which can effectively regulate the serum pain transmitter and the expression level of IL-23 and MMP-9,re-lieve lumbar pain and promote the recovery of lumbar spine function.
