基于网络化的儿童心理行为发育疾病早期预警平台的构建和效果评价

摘要
目的 构建一套集普筛性、可及性、简便性于一体的儿童心理行为发育疾病早期预警平台,并评价其应用效果和临床价值.方法 选择泸州市妇幼保健院门诊线上挂号就诊的 0～3 岁的共 16 448 名儿童为研究对象,对照组(n=8 215)常规线上挂号就诊,研究组(n=8 233)挂号时使用嵌入"儿童心理行为发育问题预警征象"的线上挂号系统.对比两组儿童的心理行为发育疾病管理率等相关指标.结果 与对照组相比,研究组儿童心理行为发育问题筛查率(33.2%vs 26.1%,P<0.001)、发育异常/偏离发现率(1.6%vs 0.5%,P<0.001)、发育异常/偏离检出率(4.7%vs 2.1%,P<0.001)、高危儿发现率(1.26%vs 0.67%,P<0.001)等指标均显著增加.结论 基于网络化的儿童心理行为发育疾病早期预警平台可显著提高儿童心理行为发育异常的早期发现和早期诊断比例,可作为适用技术进行推广.
Abstract
Objective To build a set of early warning platform with accessibility and simplicity for children's psychological and behavioral development disease screening,and evaluate its application effect and clinical value.Methods A total of 16 448 children aged 0-3 years were enrolled,who registered online in Luzhou Maternal and Child Health Hospital.The children in the control group(n=8 215)used conventional online registration,and the children in the research group(n=8 233)used online registration embedded with children's psychological behavior development warning signs.Psychological-behavioral developmental disease management rates and other related indicators between the two groups were statistically compared.Results Compared with the control group,the screening rate of children's psychological and behavioral development problems(33.2%vs 26.1%,P<0.001),developmental abnormality/deviation discovery rate(1.6%vs 0.5%,P<0.001),developmental abnormality/deviation detection rate(4.7%vs 2.1%,P<0.001),and the detection rate of high-risk children(1.26%vs 0.67%,P<0.001)were significantly higher in the research group.Conclusion The network-based early warning platform for children's psychological and behavioral development diseases can significantly improve the early detection and early diagnosis proportion of children's psychological and behavioral development abnormalities,which can be promoted as an adaptation technology.
