摘要
目的 通过有限元模型模拟测试不同坐姿下的腰椎间盘应力、内压,分析各姿势对于腰椎的负荷大小,为无症状人群日常生活的健康坐姿,及慢性腰痛(chronic low back pain,CLBP)等常见疾病的治疗提供参考.方法 将健康志愿者CT、X线片数据导入Mimics、Geomagic Studio和HyperMesh中重构建模,建立放松坐姿前倾、挺直坐姿(中立位)、放松坐姿后倾、放松坐姿(中立位)、放松坐姿(中立位)+腰部支撑共5种典型坐姿的有限元模型,再赋予相关部位接近生物实体的材料属性(弹性模量、泊松比等),通过Abaqus分析其应力分布和椎间盘内压.结果 挺直坐姿(中立位)、放松坐姿(中立位)+腰部支撑这两种姿势的腰椎间盘Mises应力、椎间盘内压相较于其他姿势明显更小.5种坐姿椎间盘内压的变化情况基本与椎间盘Mises应力峰值变化情况一致.结论 不同坐姿情况下腰椎曲度、骨盆方向、肌肉活动等都会影响腰椎负荷的分布与大小,采用正确坐姿有利于预防腰椎疾病,缓解CLBP等常见腰椎问题症状.
Abstract
Objective The stress and internal pressure of lumbar intervertebral disc under different sitting positions are simulated and tested by finite element model,and to analyze the burden of each position on lumbar spine,so as to provide reference for the healthy sitting position of asymptomatic people's daily life and the treatment of common diseases such as chronic low back pain(CLBP).Methods CT and X-ray data of healthy volunteers were imported into mics,Geomagic studio and HyperMesh for recon-struction modeling,and five typical sitting posture finite element models were established,including relaxed sitting posture forward leaning,upright sitting posture(neutral position),relaxed sitting posture backward leaning,relaxed sitting posture(neutral position),relaxed sitting posture(neutral position)+lumbar support,and then the relevant parts were given material properties close to biologi-cal entities(elastic modulus,Poisson's ratio,etc.),The stress distribution and intradiscal pressure were analyzed by ABAQUS.Re-sults Compared with other postures,the Mises stress and intradiscal pressure of lumbar intervertebral disc in neutral position of up-right sitting position and relaxed sitting position of lumbar support are significantly smaller.The changes of IDP in five sitting positions are basically consistent with the changes of Mises stress peak of intervertebral disc.Conclusions Lumbar curvature,pelvic direction,muscle activity,etc.under different sitting positions will affect the distribution and size of lumbar load.Adopting correct sitting posi-tions is conducive to prevent lumbar diseases and alleviate common symptoms of lumbar problems such as CLBP.
