摘要
目的 探讨慢性双侧腰痛年轻女护士腰椎椎旁肌的结构变化.方法 于2023年6月至8月将该院51例双侧腰痛的年轻女护士纳入为研究组,同期在该院健康体检的年轻非护理从业女性51例作为对照组.采用MRI检测受试者L2-3至L5-S1椎间盘(IVD)校正的腰椎椎旁肌横截面积(CSA)[多裂肌(MF)IVD、竖脊肌(ES)IVD、腰大肌(PM)IVD],测量MF、ES、PM的MRI信号强度.结果 两组L2-3、L3-4、L4-5、L5-S1椎间水平MFIVD、ESIVD、PMIVD差异均无统计学意义(P>0.05).研究组L2-3、L3-4、L4-5、L5-S1椎间水平MF、ES、PM的MRI信号密度均高于同椎间水平对照组,差异均有统计学意义(P<0.05);两组L5-S,椎间水平MF、ES的MRI信号密度高于其他椎间水平,差异均有统计学意义(P<0.05).研究组L2-3、L3-4、L4-5、L5-S1椎间水平ES和MF的MRI信号密度均高于同椎间水平PM的MRI信号密度(P<0.05).结论 慢性腰痛年轻女护士的腰椎椎旁肌脂肪浸润程度增加,L5-S,椎旁肌肉脂肪浸润程度高于其他水平的椎旁肌肉.
Abstract
Objective To investigate the structural changes of lumbar paravertebral muscle in young female nurses with chronic bilateral low back pain.Method From June to August 2023,51 nurses with bilateral low back pain in our hospital were included in the study group,and 51 young women who underwent physical examinations in our hospital during the same period were included in the control group.The lumbar paravertebral muscle cross-sectional area(CSA)corrected at the intervertebral disc level(L2-3 to L5-S1)of the subjects[multifidus(MF)IVD,erector spinalis(ES)IVD,psoas major(PM)IVD]was detected by MRI,and the MRI sig-nal intensity of MF,ES,and PM was measured.Results There was no statistically significant difference in MFIVD,ESIVD,and PMIVD at the L2-3,L3-4,L4-5,and L5-S,intervertebral level between the two groups(P>0.05).The signal densities of MF,ES,and PM 的 MRI at the intervertebral level of L2-3,L3-4,L4-5,and L5-S,in the study group were higher than those in the control group at the same intervertebral level,and the differences were statistically significant(P<0.05).The signal density of MF and ES的 MRI at L5-S1 intervertebral level was higher than that at other intervertebral levels in two groups,and the differences were signifi-cant(P<0.05).The signal density of ES and MF 的 MRI at intervertebral level of L2-3,L3-4,L4-5 and L5-S1 in the study group was higher than that of PM 的 MRI at intervertebral level(P<0.05).Conclusion In young female nurses with chronic low back pain,the degree of lumbar paravertebral muscle fat infiltration increased,and the degree of L5-S1 paravertebral muscle fat infiltration was higher than that of other paravertebral muscles.
