Objective To explore the predictive value of microRNA-126(miR-126)and immune function indexes in blood and bronchoalveolar lavage fluid(BALF)for prognosis of patients with severe lung infection in Intensive Care Unit(ICU).Methods A total of 96 patients with severe lung infection treated in ICU were enrolled as severe group between March 2019 and March 2021,while other 88 patients without severe lung infection during the same period were enrolled as non-severe group.According to 28d prog-nosis,patients in severe group were divided into survival group and dead group.The levels of miR-126 in serum and BALF,ratios of cellular immune indexes CD3+,CD4+,CD8+T lymphocytes and humoral immune indexes[immunoglobulins(IgG,IgM and IgA)]in different groupswerecompared.Results ThelevelsofmiR-126 inserumand BALF,CD3+ and CD4+/CD8+ inperipheralblood,serum IgG and IgM in severe group were lower than those in non-severe group,while score of acute physiological and chronic health e-valuation Ⅱ(APACHE Ⅱ)was higher than that in non-severe group(P<0.05);Pearson correlation analysis showed that levels of miR-126 in serum and BALF,and CD4+/CD8+ were negatively correlated with APACHE II score(r =-0.432,-0.459 and-0.538,P<0.05);the levels of miR-126 in serum and BALF,CD3+ and CD4+/CD8+ in peripheral blood,and serum IgG in sur-vival group were higher than those in death group(P<0.05);receiver operating characteristic curve(ROC)showed that the com-bined prediction of miR-126 and immune function indexes in blood and BALF was 0.894,and 95%CI was 0.814-0.947,which were higher than each single index.Conclusion The miR-126 in blood and BALF,and immune function indexes are closely related to severe pulmonary infection,which is an important indicator to predict the prognosis of patients.
