The Measurement and Clinical Significance of the Cross Sectional Area of the Anterior Opening of the Vidian Canal in Patients with Allergic Rhinitis with HDCT
Objective To investigate the differences and clinical significance of changes in the cross-sectional area of the exter-nal pterygoid canal under high-resolution CT in patients with allergic rhinitis(AR),AR with sinusitis,AR with sinusitis and nasal polyps.Methods 80 patients including normal control group(20 patients)and allergic rhinitis patients(60 patients)in our de-partment were randomly selected from October 2018 to May 2022.80 patients were divided into 4 groups with 20 cases in each group:normal group(group Ⅰ),simple allergic rhinitis group(group Ⅱ),allergic rhinitis with sinusitis group(group Ⅲ),and the al-lergic rhinitis with sinusitis and nasal polyps group(group Ⅳ).The anterior opening of the vidian canal was located by high resolution sinus coronal CT,and the anterior opening of the vidian canal was classified by PASS software system.The distance and cross-section-al area of the anterior opening of both vidian canal were measured by imaging tools,and the measurement results were statistically ana-lyzed.Results(1)The pterygoid ducts were round or oblate in the coronal plane of the sinus,with varying calibre sizes,and were typed and counted according to the pterygoid duct typing method,with no significant difference in pterygoid duct typing between groups by statistical analysis x2=2.470,P＞0.05.(2)Using the polygon tool in the diagnostic imaging workstation,measurement of the anterior pterygoid canal area was performed.The q-test of the mean pterygoid canal external orifice area in the four groups showed that compared with group Ⅰ,there were significant differences in groups Ⅲ and Ⅳ(P＜0.05);compared with group Ⅰ,there were no sig-nificant differences in group Ⅱ(q=2.75,P＞0.05);compared with Ⅲ,there were significant differences in group Ⅳ(q=3.38,P＜0.05).The trend of increasing the area of the anterior pterygoid canal was the most obvious in group Ⅳ;there was a significant difference(P＜0.05)in the comparison of VC distance between group Ⅱ,group Ⅲ and group Ⅳ and group Ⅰ;group Ⅱ was com-pared with group Ⅰ,q=3.03,P＜0.05;group Ⅲ was compared with group Ⅰ,q=4.83,P＜0.01;group Ⅳ was compared with group Ⅰ,q=7.09,P＜0.01;there was no significant difference between group Ⅳ and group Ⅲ(q=2.25,P＞0.05).Conclusion There were differences in the distance between the anterior pterygoid ducts and the cross-sectional area of the anterior pterygoid ducts in patients with allergic rhinitis(AR),AR with sinusitis,AR with sinusitis and nasal polyps under high-resolution CT,while pa-tients with AR with sinusitis and nasal polyps had the greatest impact on the cross-sectional area of the pterygoid ducts under high-res-olution CT,with a tendency to expand,providing anatomical reference for surgery around the pterygoid ducts.
