基于质谱代谢组学分析冠状动脉狭窄和钙化对循环代谢物的影响

冠心病是一种常见的心血管疾病,严重威胁人类健康.冠状动脉钙化在冠心病患者中非常普遍,钙化的进展与不良心血管疾病预后息息相关.本研究共召集164 位患者,91 名冠心病患者中钙化人群比例为68.13%,其中62 人发生了冠脉钙化,29 人无钙化.通过对召集的患者进行年龄、性别、身体质量指数匹配,最终共入组68 位患者进行血浆代谢组学研究,包括非冠心病(n=29)和冠心病(n=29)患者以及冠心病中血管钙化(n=12)和无钙化患者(n=12),以探究冠状动脉狭窄和钙化对循环代谢物的影响.超高效液相色谱-高分辨质谱联用技术共鉴定出血浆中的101 个内源性代谢物,t检验结果表明,冠心病与非冠心病患者中含量有显著性差异的代谢物有12 个,钙化与非钙化患者中含量有显著性差异的代谢物有2 个.将基于稀疏正则化的子抽样-支持向量机方法所筛选到的代谢物和t检验中含量具有显著性差异的代谢物一起进行富集分析和代谢通路分析,结果表明,冠状动脉狭窄是影响循环代谢的主要因素,主要影响丙氨酸、天冬氨酸和谷氨酸代谢为主的氨基酸代谢、亚麻酸代谢、甘油磷脂代谢和三羧酸循环;冠状动脉狭窄基础上的血管钙化对循环代谢影响较小,只有溶血磷脂酰乙醇胺(18∶2)在钙化组中显著上调.
Metabolomic Analysis Based on Mass Spectrometry of the Impact of Coronary Artery Stenosis and Calcification on Circulating Metabolites
Coronary heart disease is a common cardiovascular disease,which seriously threatens human health.Coronary artery calcification is widespread in patients with coronary heart disease,and the progression of calcifi-cation is closely related to the prognosis of adverse cardiovascular disease.A total of 164 patients are recruited in this study.Among the 91 patients with coronary heart disease,the proportion of calcification was 68.13%,of which 62 have coronary artery calcification and 29 have no calcification.By matching age,sex,and body mass in-dex,a total of 68 patients are enrolled for plasma metabolomics study,including non-coronary heart disease pa-tients(n=29)and coronary heart disease(n=29),and patients with vascular calcification in coronary heart disease(n=12)and non-calcification(n=12),to explore the effects of coronary artery stenosis and calcifica-tion on circulating metabolites.A total of 101 endogenous metabolites in plasma are identified by ultra-high per-formance liquid chromatography-high resolution mass spectrometry.The results of the t-test show that there are 12 metabolites with significant differences between patients with coronary heart disease and non-coronary heart disease,and 2 metabolites with substantial differences between patients with calcification and non-calcification.The metabolites selected by the sparse regularization variable selection with the subsampling-support vector ma-chine method and the metabolites with significant differences in content in the t-test were analyzed together for enrichment analysis and metabolic pathway analysis.The results indicate that coronary artery stenosis is the main factor affecting the circulation metabolism,which mainly affects the metabolism of amino acids including alanine,aspartic acid and glutamic acid,and the metabolism of linolenic acid,glycerophospholipid metabolism and tricar-boxylic acid metabolism.Vascular calcification based on coronary artery stenosis has little effect on circulatory metabolism,and only LPE(18∶2)is significantly up-regulated in the calcification group.
