摘要

目的 评估不同年龄段人群的颧颌缝成熟阶段及其与年龄和性别的关系.方法 本研究为回顾性临床研究,收集武汉大学人民医院6~22岁的517名受试者的锥形束CT扫描图像,使用Angelieri等学者分析颧颌缝成熟阶段的五级方法进行评估.采用SPSS 27.0软件进行数据分析.结果 颧颌缝按其发育阶段的成熟度分为A、B、C、D、E 5期.分期A在6~12岁的儿童中均有出现,主要集中在6~7岁的年龄段,在6岁的男童中分期为A的占比为88.9%,在6岁女童中分期为A的占比为76.9%,在7岁的男童中分期为A的占比为61.5%,在7岁女童中分期为A的占比为60.0%;分期B在6~18岁的年龄段均存在,主要集中在8~12岁的年龄段,在10岁儿童中占比最高(男童90.5%,女童72.2%);分期C在9岁及以上的年龄中均有分布,主要集中在13~15岁年龄段,在14岁男童(50.0%)和13岁女童(36.4%)占比最高;D、E分期主要集中在16岁及以上的年龄段,16岁及以上的青少年和年轻成人中,50%以上处于D、E期.统计学分析显示,颧颌缝成熟度与年龄相关,与性别不相关.同一年龄段人群的颧颌缝成熟阶段有很大的个体差异.结论 使用锥形束CT图像对颧颌缝成熟阶段的个体化评估,为骨性Ⅲ类患者前方牵引的适应证与矫治时机的选择提供参考.

Abstract

Objective To assess the maturation stages of the zygomaticomaxillary suture in different age groups and their relationship with age and gender.Methods This retrospective clinical study collected cone-beam CT scan images of 517 subjects aged 6 to 22 years from Renmin Hospital of Wuhan University.The five-stage method proposed by Angelieri.was used to evaluate the maturation stages of the zygomaticomaxillary suture.Data analysis was performed using SPSS 27.0 software.Results Stage A appeared in children aged 6 to 12 years,primarily in the 6 to 7-year age group.Among 6-year-old boys,88.9%were at stage A,and among 6-year-old girls,76.9%were at stage A.In 7-year-olds,61.5%of boys and 60.0%of girls were at stage A.Stage B was present in individuals aged 6 to 18 years,mainly between 8 and 12 years old.It was most prevalent among 10-year-olds(90.5%in boys and 72.2%in girls).Stage C was distributed among those aged 9 years and above,with a concentration in the 13 to 15-year age group,peaking at 50.0%in 14-year-old boys and 36.4%in 13-year-old girls.Stages D and E were mainly seen in individuals aged 16 years and above,with over 50%of adolescents and young adults in this age group being at stages D or E.Statistical analysis showed that the maturation stage of the zygomaticomaxillary suture was related to age but not to gender.There was significant individual variability in the maturation stages within the same age group.Conclusions Individualized assessment of zygomaticomaxillary suture maturation stages using cone-beam CT images provides a reference for determining the indications and optimal timing for orthodontic treatment in patients with skeletal Class Ⅲ malocclusion.
