Correlation between rectus abdominis muscle thickness measured by ultrasound and cachexia stage in patients with malignant tumors

目的 应用超声检测腹直肌厚度,分析其与恶性肿瘤患者恶液质分期的相关性.方法 选取我院住院复查的恶性肿瘤患者68例为肿瘤组,根据恶液质评分表分为无恶液质期10例、恶液质前期16例、恶液质期32例和恶液质难治期10例;另选健康体检者41例为健康对照组.记录入组者基线资料和实验室指标(白细胞计数、血红蛋白、白蛋白);应用超声测量腹直肌厚度、腹部正中皮下脂肪厚度,计算腹直肌厚度与体质量指数(BMI)比值(腹直肌厚度/BMI),比较肿瘤组与健康对照组、肿瘤组各恶液质分期患者上述参数的差异,分析腹直肌厚度与恶液质分期的相关性.结果 肿瘤组与健康对照组体质量、白细胞计数、血红蛋白、白蛋白、腹直肌厚度、腹直肌厚度/BMI比较,差异均有统计学意义(均P<0.05);腹部正中皮下脂肪厚度比较,差异无统计学意义.肿瘤组恶液质难治期与恶液质前期、恶液质难治期与无恶液质期、恶液质期与无恶液质期患者体质量、腹直肌厚度比较,差异均有统计学意义(均P<0.05);肿瘤组各恶液质分期患者白细胞计数、血红蛋白、白蛋白比较,差异均有统计学意义(均P<0.05).相关性分析显示,腹直肌厚度与恶液质分期呈中度负相关(rs=-0.521,P<0.001),与体质量呈中度正相关(rs=0.435,P<0.001).结论 超声所测腹直肌厚度随恶性肿瘤恶液质程度的增加而减小,应用超声可准确评估患者肌肉减少,有望成为评估恶液质严重程度的新方法.
Objective To analyze the correlation between the rectus abdominis muscle thickness measured by ultrasound and the cachexia stage in patients with malignant tumors.Methods sixty-eight patients with malignant tumors who reexamined in our hospital were selected as tumor group.According to the cachexia score table,the patients were divided into no cachexia group(10 cases),pre-cachexia group(16 cases),cachexia group(32 cases)and cachexia refractory group(10 cases),and 41 healthy volunteers were selected as the healthy group.Baseline data and laboratory indicators(white blood cell count,hemoglobin and albumin)were recorded.The rectus abdominis muscle thickness and abdominal median subcutaneous fat thickness were measured by ultrasound,and the ratio of rectus abdominis muscle thickness to body mass index(BMI)was calculated.The differences of above parameters between the tumor group and the healthy group,different cachexia stage in tumor group were compared.The correlation between rectus abdominis muscle thickness and the cachexia stage was analyzed.Results There were significant differences in body weight,white blood cell count,hemoglobin,albumin,rectus abdominis muscle thickness and the ratio of rectus abdominis muscle thickness to BMI between the tumor group and the healthy group(all P<0.05).There was no difference in the abdominal median subcutaneous fat thickness.There were differences in the rectus abdominis muscle thickness between cachexia refractory group and pre-cachexia group,cachexia refractory group and no cachexia group,cachexia group and no cachexia group(all P<0.05).There were significant differences in white blood cell count,hemoglobin and albumin at different cachexia stage in tumor group(all P<0.05).Correlation analysis showed that the rectus abdominis muscle thickness was negatively correlated with the cachexia stage(rs=-0.521,P<0.001),and moderately positively correlated with body mass(rs=0.435,P<0.001).Conclusion The rectus abdominis muscle thickness measured by ultrasound in patients with malignant tumors decreases with the severity of cachexia increases.Ultrasound can accurately assess muscle wasting and may become a new method for evaluating the severity of cachexia.
