Objective To investigate the efficacy of the supercapsular percutaneously(SuperPATH)appraoch hip ar-throplasty for femoral neck fracture on hemiplegic side.Methods Nineteen cases of femoral neck fractures on hemi-plegia side were treated by SuperPATH approach hip arthroplasty,the complications,usage of crutches,prosthesis po-sition were recorded.The function of hip joint was evaluated by Harris score.Results Successful operations was com-pleted in 18 cases,1 case was changed to conventional posterolateral approach because of the reduction difficulty dur-ing operation(all indexes were not examined).All patients were followed up for 3~36(19.4±11.3)months.After operation,1 patient developed severe hypokalemia and hypoproteinemia,who improved after being transferred to ICU;thrombosis of intermuscular vein of calves was found in 1 case,who recovered within 1 month after anticoagulation therapy.There were no postoperative complications such as joint instability or dislocation,pulmonary embolism,femo-ral fracture and infection.Three months after the operation,13 cases could walk independently,3 cases could walk with single crutch,2 cases still needed wheelchair.At the last follow-up,the position of prostheses were all good,with-out loosening;Harris scores were 54~92(83.1±11.0)points,of which 7 cases were excellent,8 cases good,1 fair and 2 poor,the excellent-good rate was 15/18.Conclusions In the treatment of the femoral neck fracture on hemi-plegia side,the SuperPATH approach hip arthroplasty can effectively prevent the dislocation of the hip joint by protec-ting the posterior articular capsule and external rotation muscle tissue.There are no restrictions on the choice of pros-thesis,no special requirements on the angle of prosthesis placement,with satisfactory clinical efficacy.
