Objective To compare the efficacy of hemi-hip arthroplasty(HHA)and total hip arthroplasty(THA)for elderly femoral neck fracture.Methods The 133 elderly patients with femoral neck fractures were divided into HHA group(63 patients were treated with HHA)and THA group(70 patients were treated with THA),according to the disease coditions and patient′s wishers,each group was divided into two age periods including 65~80 years old and a-bove 80 years old.The intraoperative bleeding volume,surgical time,hip joint range of motion,and gait indicators(stride speed,stride frequency and stride length)in both groups were recorded.Harris scale was used to evaluate hip joint function.Results All patients were followed up for12~24(18.21±3.40)months.①Intraoperative blood loss and operation time:there were no statistical differences for different age periods in two groups(P>0.05);however,for different age periods,the two indicators in THA group were longer(more)than those in HHA group(P<0.05).②Hip joint range of motion and gait indicators:at the last follow-up,the two indexes of 65~80 years old age period were better than those of above 80 years old period in two groups(P<0.05);there were no statistical differences for differ-ent age periods between two groups(P>0.05).③ Harris scale:at the last follow-up,the index of 65~80 years old period was higher than that above 80 years old period in two groups(P<0.05);the different age periods of THA group were all higher than those of the HHA group(P<0.05).Conclusions Using HHA and THA for treatment of elderly femoral neck fractures,HHA has advantages over THA in terms of operation time and intraoperative blood loss,while THA has advantages over HHA in hip joint function recovery.Therefore,THA is recommended for patients with longer life expectancy,high activity or combined with hip osteoarthritis,while HHA is recommended for advanced aged patients with relatively poor physical conditions.
