Objective To investigate the effect of modified anterolateral combined posteromedial approaches internal fixation on tibial plateau fracture.Methods The 16 patients with Schatzker type Ⅴ and Ⅵ tibial plateau fractures were treated with modified anterolateral and posteromedial approaches internal fixation.Fracture reduction,healing situation,knee range of motion,HSS score and complications were recorded.Results All patients were followed up for 12~18 months.Postoperative radiographs showed good alignment of all fractures.There were no complications such as infection,necrosis of the surrounding skin and deep venous thrombosis of lower extremity.At 6 weeks after surgery,radiographs showed callus was formation,and 3 months after surgery,radiographs showed fractures were all healing,without obvious pain in the affected limb,and all patients could walk with full bearing-weight.At 12 months after the operation,X-ray films showed good fracture healing and alignment.Knee flexion was 110°~135°(122.0°±5.1°),extension was 0°~10°(5.5°±3.1°);HSS scores were 88~94(90.5±1.7)points.Conclusions Modi-fied anterolateral combined posterolateral approaches internal fixation on tibial plateau fracture can effectively reduce surgical complications and facilitate early functional exercise with satisfactory results,avoiding position change,full exposure of visual field and less soft tissue stripping of fracture end.
