颈椎矢状面平衡参数与颈椎间盘退变的相关性

目的 探讨颈椎矢状面平衡参数与颈椎间盘退变程度的相关性.方法 在150例颈椎病患者的颈椎侧位X线片上测量C2~7椎间盘节段的退变程度以及颈椎矢状面平衡参数[C2~7 Cobb角、C0~7 Cobb角、C2~7矢状面垂直距离(C2~7SVA)、T,倾斜角(T1S)及T1S-C2~7 Cobb角],分析颈椎间盘退变程度与性别、年龄及颈椎矢状面平衡参数的相关性.根据C2~7椎间盘退变程度分为A组(退变程度较轻,总评分<3分,56例)和B组(退变程度较重,总评分≥3分,94例),比较两组矢状面平衡参数的差异及组内参数之间的相关性.结果 C2~7椎间盘退变程度总评分不同年龄之间比较差异有统计学意义(P<0.01),不同性别、颈椎曲度之间比较差异均无统计学意义(P>0.05).T1S与C2~7颈椎间盘退变程度总评分呈正相关(P<0.05),而其余矢状面平衡参数与之无相关性(P>0.05).T1S B组高于A组(P<0.05);其余矢状面平衡参数两组比较差异无统计学意义(P>0.05).两组内C2~7 Cobb角与T1S均呈正相关(P<0.01),与T1S-C2~7Cobb角均呈负相关(P<0.01);C2~7 SVA 与 T1S-C2~7 Cobb 角均呈正相关(P<0.01);A 组内 C2~7 Cobb 角与 C2~7 SVA 呈负相关(P<0.01),而B组内两者无相关性(P>0.05);B组内T1S与C2~7 SVA呈正相关(P<0.01),而A组内两者无相关性(P>0.05);两组内T1S与T1S-C2~7 Cobb角均无相关性(P>0.05).结论 颈椎间盘退变程度与年龄密切相关;在常用颈椎矢状面平衡参数中,T1S与颈椎间盘整体退变呈正相关,颈椎间盘退变程度较重的患者中T1S与C2~7 SVA呈正相关.
Objective To investigate the correlation between cervical sagittal balance parameters and cervical disc de-generation degree.Methods The degeneration degree of C2~7 intervertebral disc and the parameters of sagittal bal-ance of C2~7 Cobb angle,C0~7 Cobb angle,vertical distance of C2~7 sagittal plane(C2~7 SVA),T,slope(T1S)and the value of T,S-C2~7 Cobb angle were measured on the lateral radiographs of 150 patients with cervical spondylosis.The correlation between the cervical disc degeneration degree and gender,age and cervical sagittal plane parameters was analyzed.According to the C2~7 cervical disc degeneration degree,they were divided into group A(mild degree of degeneration,total scores<3 points,56 cases)and group B(severe degree of degeneration,total scores ≥3 points,94 cases).The difference of sagittal plane parameters between the two groups and the correlation between parameters in the two groups were compared.Results There were statistical differences in the total scores of C2~7 cervical disc de-generation among different ages(P<0.01),but there were no statistical differences in gender and cervical curvature(P>0.05).T1S:it was positively correlated with the total scores of C2~7 cervical disc degeneration(P<0.05),which was not correlated with the other sagittal plane parameters(P>0.05).T1S of group B was higher than group A(P<0.05);there were no statistical differences in other sagittal plane parameters between the two groups(P>0.05).In the two groups,C2~7 Cobb angles were positively correlated with T,S(P<0.01),and negatively correlated with T1S-C2~7 Cobb angles(P<0.01).C2~7 SVA was positively correlated with T1S-C2~7 Cobb angles(P<0.01);there was negative correlation between C2~7 Cobb angle and C2~7 SVA in group A(P<0.01),but no correla-tion between them in group B(P>0.05).There was positive correlation between T1 S and C2~7 SVA in group B(P<0.01),but no correlation between T1S and C2~7 SVA in group A(P>0.05).There was no correlation between T1S and T1S-C2~7 Cobb angle in two groups(P>0.05).Conclusions The cervical disc degeneration degree is closely related to age.Among the commonly used cervical sagittal plane parameters,T1S is positively correlated with overall cervical disc degeneration,and in patients with severe cervical disc degeneration,T1S is positively correlated with C2~7 SVA.
