Objective To compare the efficacy of unilateral and bilateral puncture percutaneous kyphoplasty(PKP)in the treatment of osteoporotic vertebral compression fracture(OVCF).Methods The 117 patients with OVCFs trea-ted by PKP were divided into unilateral group(58 cases were treated with unipedicular puncture PKP)and bilateral group(59 cases were treated with bipedicular puncture PKP).The surgical status,pain VAS,Cobb angle and verte-bral anterior margin height compression ratio were compared between the two groups.Results All patients were fol-lowed up for 5~11(7.7±2.1)months.Operative time,intraoperative fluoroscopy frequency and bone cement injec-tion volume were shorter(less)in unilateral group than those in bilateral group(P<0.01).Bone cement leakage oc-curred in 11 cases(19.00%)of unilateral group and 8 cases(13.56%)of bilateral group.There was no statistical difference in the bone cement leakage rate between the two groups(P>0.05).Pain VAS,Cobb angle and vertebral anterior margin height compression ratio were all improved at 2 d and 3 months after surgery,compared with the preop-eration(P<0.05).There were no statistical differences in pain VAS and Cobb angles between the two groups on the 2nd day and 3 months after surgery(P>0.05).Vertebral anterior margin height compression ratio:There was no sta-tistical difference between the two groups on the 2nd day after surgery(P>0.05),and the bilateral group was lower than the unilateral group at 3 months postoperation(P<0.01).Conclusions Both unilateral and bilateral puncture PKP can achieve satisfactory and similar clinical effect in the treatment of OVCFs,unilateral puncture has shorter op-eration time,fewer intraoperative fluoroscopy frequency and less bone cement injection volume,while bilateral punc-ture has better retention rate of vertebral anterior margin height.
