摘要
目的 探讨弧形髋臼周围截骨术(CPO)治疗成人髋关节发育不良的疗效.方法 采用CPO治疗17 例髋关节发育不良患者(20 髋).记录外侧中心边缘角(LCEA)、Sharp角、术后并发症发生情况,采用Harris评分及疼痛VAS评分评估疗效.结果 患者均获得随访,时间8～66 个月.末次随访时,Harris评分、疼痛VAS评分、LCEA、Sharp角均较术前改善(P＜0.01).术后3 例出现股外侧皮神经支配区麻木,3 例耻骨截骨面延迟愈合,1 例切口浅表感染,未发生关节内截骨、重大血管神经损伤等并发症.结论 CPO治疗成人髋关节发育不良临床疗效好,可以明显缓解患者髋关节疼痛症状并延缓骨关节炎的进展.
Abstract
Objective To investigate the efficacy of curved periacetabular osteotomy(CPO)in the treatment of adult developmental dysplasia of the hip(DDH).Methods The 17 patients with DDH were treated by CPO(20 hips).The lateral center-edge angle(LCEA),Sharp angle and complications were recorded.The therapeutic effect was evaluated by Harris hip score and pain VAS.Results All patients were followed up for 8～66 months.At the last follow-up,Harris scores,pain VAS,LCEA and Sharp angle were improved than the preoperation(P＜0.01).The lateral femoral cutaneous innervation area was numb in 3 cases.Delayed healing of the pubic osteotomy surface occurred in 3 cases,1 case had superficial incision infection,there were no complications such as intra-articular osteotomy and major vas-cular and nerve injuries.Conclusions CPO has good clinical efficacy in the treatment of DDH,which can significant-ly relieve the symptoms of hip pain,and delay the progression of osteoarthritis.
