Objective To investigate the clinical effect of manual reduction and plaster external fixation in the treat-ment of Bado type Ⅲ Monteggia fracture in children. Methods The 28 cases of Bado type Ⅲ Monteggia fractures were treated with manual reduction and plaster external fixation.Fracture reduction, complications occurrence, fracture healing and joint motion were recorded.Kim score system was used to evaluate elbow joint function. Results All the patients were followed up for 3~6 months. The humeroradial joint was successfully reduced in all patients. No vascu-lar or nerve injury occurred during treatment. There was no complications such as heterotopic ossification and nonunion after treatment.Fracture healing time was 6~12 weeks. At the last follow-up,X-film radiographs showed a good rela-tionship between humeroradial joint, and ulnar fracture line was blurred, the fractures healed well. The elbow joint on the affected side was extended with 0°~10°,the flexion was 135°~145°,the forearm was pronated 75°~90°,prona-tion was80°~90°,and there was no significant difference, compared with the healthy side.Kim score system was used to evaluate elbow joint function, and all 28 cases were excellent. Conclusions In the treatment of Bado type Ⅲ Monteggia fracture in children, manipulative reduction and plaster external fixation have less trauma, good fracture re-duction,few complications and good recovery of joint function.
