股骨近端防旋髓内钉治疗老年股骨转子间骨折

摘要
目的 探讨股骨近端防旋髓内钉(PFNA)治疗老年股骨转子间骨折的疗效.方法 采用PFNA 治疗27例老年股骨转子间骨折患者.记录手术情况、骨折愈合时间、疼痛 VAS 评分、髋关节功能Harris评分、并发症发生情况.结果 手术时间50～90(79.8±11.6)min,术中出血量60～100(80.5±10.2)ml,术中透视6～15(7.5±2.3)次,住院时间14～29(20.4±3.7)d.患者均获得随访,时间6～15(12.5±1.2)个月.术后1 周疼痛 VAS 评分1～3(1.3±0.2)分.骨折均愈合,时间 15～21(16.7±2.1)周.术后 6 个月髋关节功能Harris评分76～95(86.2±9.3)分,其中优12 例,良9 例,可6 例,优良率为21/27.术后未发生血栓、异位骨化以及螺旋刀片切割、松动或断裂等情况,1 例发生髓内钉远端股骨骨折.结论 PFNA治疗老年股骨转子间骨折,手术操作简单,创伤小,术后功能恢复快.
Abstract
Objective To investigate the effect of proximal femoral nail anti-rotation(PFNA)on elderly patients with intertrochanteric fracture of femur.Methods The 27 elderly patients with femoral intertrochanteric fractures were treated with PFNA.The operation situation,fracture healing time,pain VAS,Harris score of hip function and complica-tions were recorded.Results The operative time was 50～90(79.8±11.6)min,the intraoperative blood loss was 60～100(80.5±10.2)ml,the intraoperative fluoroscopy frequency was 6～15(7.5±2.3)times,and the hospital stay was 14～29(20.4±3.7)d.All patients were followed up for 6～15(12.5±1.2)months.Pain VAS of the first week after surgery was 1～3(1.3±0.2)scores.All the fractures healed within 15～21(16.7±2.1)weeks.At 6 months after operation,the Harris scores of hip function were 76～95(86.2±9.3)points,of which 12 cases were ex-cellent,9 good,6 fair,with an excellent-good rate of 21/27.There were no cases of thrombus,ectopic ossification,and cutting,loosening or fracture of spiral blade,etc.One case suffered from distal femoral fracture of intramedullary nail.Conclusions PFNA in the treatment of elderly intertrochanteric fracture of femur has simple operation,less trauma and fast postoperative functional recovery.
