新型组合式锁定钢板治疗股骨转子下骨折的疗效

摘要
目的 探讨新型组合式锁定钢板治疗股骨转子下骨折的疗效.方法 将46 例股骨转子下骨折患者根据治疗方式不同分为观察组(采用新型组合式锁定钢板固定治疗,23 例)和对照组(采用股骨重建髓内钉固定治疗,23 例).记录手术情况、骨折愈合时间、并发症发生情况,采用髋关节Harris评分评价疗效.结果 患者均获得随访,时间10～24 个月.手术时间、术中出血量、住院时间、骨折愈合时间观察组均短(少)于对照组(P＜0.05).术后1、3、6 个月髋关节功能Harris评分两组比较差异均无统计学意义(P＞0.05).对照组 1例因股骨外侧壁粉碎性骨折,术后骨不连,二期行更换内固定、自体植骨治疗后骨折愈合.结论 新型组合式锁定钢板治疗股骨转子下骨折具有术中出血少及手术时间、住院时间、骨折愈合时间短等优点.
Abstract
Objective To investigate the efficacy of new type combined locking plate in the treatment of femoral sub-trochanteric fracture.Methods The 46 patients with femoral subtrochanteric fractures were divided into observation group(23 cases were treated with new combined locking plate fixation)and control group(23 cases were adopted with femoral reconstruction intramedullary fixation).The operation conditions,fracture healing time and complications were recorded,and the therapeutic effect was evaluated by Harris score of hip joint.Results All patients were followed up for 10～24 months.Operation time,intraoperative blood loss,hospital stay and fracture healing time in observation group were all shorter(less)than those in control group(P＜0.05).At 1,3,6 months postoperation,Harris scores of hip joint function had no statistical differences between two groups(P＞0.05).In the control group,1 patient was suf-fered from comminuted fracture of the femoral lateral wall,there was nonunion after operation,the fracture healed after replacement of internal fixation and autogenous bone grafting in the second stage Conclusions In the treatment of femoral substrochanteric fracute,the new type combined locking plate has the advantages of less intraoperative bleed-ing,operation time,hospital stay time,and shorter fracture healing time.
