摘要
目的 探讨TightRope纽扣钢板弹性内固定治疗髌骨骨折的疗效.方法 采用TightRope纽扣钢板弹性内固定治疗29 例髌骨骨折患者.记录骨折复位情况、术后并发症发生情况、骨折愈合时间、膝关节活动度.采用Bostman评分评价髌骨功能恢复情况,采用Lysholm评分评价膝关节功能恢复情况.结果 患者均获得随访,时间12～22 个月.骨折解剖复位26 例,功能复位3 例.术后无切口感染及周围疼痛、伸膝无力、内固定移位、骨关节炎等并发症发生.骨折均愈合,时间 10～16 周.末次随访时,膝关节屈曲 115°～135°,伸直0°～15°;采用Bostman评分评价髌骨功能恢复情况:优20 例,良9 例,优良率29/29;采用Lysholm评分评价膝关节功能恢复情况:优22 例,良7 例,优良率29/29.结论 TightRope纽扣钢板弹性内固定治疗髌骨骨折,既保证了骨折复位的坚固与可靠,又保留了髌韧带的生理长度,操作简单安全,患者术后膝关节功能恢复良好,且无需二次手术取出内固定,疗效满意.
Abstract
Objective To investigate the efficacy of TightRope button plate elastic internal fixation in the treatment of patellar fractures.Methods The 29 patients with patellar fractures were treated with TightRope button plate elastic internal fixation.The status of fracture reduction,postoperative complications,fracture healing time and knee joint range of motion were recorded.The Bostman score was used to evaluate the recovery status of patellar function,and the Lysholm score was used to evaluate knee joint function recovery.Results All patients were followed up for 12～22 months.There were 26 cases of anatomical reduction and 3 cases of functional reduction.There were no postoperative complications such as incision infection,peripheral pain,weakness in knee extension,internal fixation displacement or osteoarthritis.The fractures were all healed,taking 10～16 weeks.At the last follow-up,the knee joint was flexed 115°～135° and straightened 0°～15°;using the Bostman score to evaluate patellar fracture function:20 cases were excellent,9 cases were good,and the excellent-good rate was 29/29;Lysholm score was used to evaluate the recovery of knee joint function:22 cases were excellent,7 cases were good,and the excellent-good rate was29/29.Conclusions The treatment of patellar fracture by TightRope button plate elastic internal fixation not only ensures the firm and reliable fracture reduction,but also retains the physiological length of the patellar ligament.The operation is simple and safe,and the patient's knee function recovers well after surgery,and the internal fixation is not need for second re-moval operation,with satisfactory effect.
