摘要
目的 探讨髌骨爪联合钛克氏针治疗多类型髌骨骨折的疗效.方法 采用髌骨爪联合钛克氏针治疗39 例多类型髌骨骨折患者.记录手术情况、骨折愈合时间、并发症和膝关节活动度,采用Bostman评分标准评价治疗效果.结果 患者均获得随访,时间6～36 个月.手术时间35～115(61.28±18.04)min,术中出血量2～100(14.79±17.92)ml,切口长度6～10(8.03±0.76)cm.骨折均愈合,时间8～16(11.03±1.67)周.1例切口感染,2 例发生创伤性关节炎.未发生髌骨爪松动脱落、钛克氏针退针断裂、骨折再移位、血栓形成、压疮、肺炎等并发症.末次随访时膝关节活动度:屈曲105°～135°,伸直0°～10°;采用Bostman评分评价疗效:优33 例,良6 例,优良率100%.结论 髌骨爪联合钛克氏针治疗多类型髌骨骨折临床效果满意,有手术操作简单、并发症少、术后膝关节功能恢复好等优点.
Abstract
Objective To investigate the clinical effect of patellar claw combined with titanium Kirschner wire in the treatment of multiple types of patellar fracture.Methods The 39 patients with multiple types of patellar fractures were treated by patellar claw combined with titanium Kirschner wire.The operative condition,healing time,complica-tions and knee range of motion were recorded.Bostman score was used to evaluate the therapeutic effect.Results All patients were followed up for 6～36 months.The operative time was 35～115(61.28±18.04)min,the intraoperative blood loss was 2～100(14.79±17.92)ml,and the incision length was 6～10(8.03±0.76)cm.The healing time was 8～16(11.03±1.67)weeks.Incision infection occurred in 1 case and traumatic arthritis occurred in 2 cases.There were no complications such as patellar claw loosening,fracture of titanium Kirschner wire,redisplacement of fracture,thrombosis,pressure sore and pneumonia.At the last follow-up,knee range of motion was flexion with 105°～135°,and extension with 0°～10°.Bostman score was used to evaluate the curative effect:33 cases were excellent,6 cases were good,and the excellent and good rate was100%.Conclusions Patellar claw combined with titanium Kir-schner wire have satisfactory clinical effect on multiple types of patellar fracture,with simple operation,few complica-tions,good postoperative knee function recovery.
