摘要
目的 探讨预断式带孔克氏针张力带治疗老年髌骨骨折的疗效.方法 将70 例老年髌骨骨折患者按照随机数字表法分为观察组(采用预断式带孔克氏针张力带治疗,35 例)和对照组(采用传统AO克氏针张力带治疗,35 例).比较两组术中出血量、手术时间、首次下床活动时间、住院时间、骨折愈合时间、术后并发症发生情况、膝关节活动度.采用Böstman标准评价膝关节功能.结果 患者均获得随访,时间 7～14 个月.术中出血量、手术时间、住院时间观察组均少(短)于对照组(P＜0.05);首次下床活动时间、骨折愈合时间两组比较差异均无统计学意义(P＞0.05).术后并发症发生率观察组低于对照组(P＜0.05).术后6 个月采用Böstman标准评价的膝关节功能优良率观察组高于对照组(P＜0.05).末次随访时膝关节屈曲活动度观察组大于对照组(P＜0.05).结论 预断式带孔克氏针张力带治疗老年髌骨骨折,具有术中出血量少、手术时间短、住院时间短、术后并发症少、患者膝关节功能恢复好的优点.
Abstract
Objective To investigate the efficacy of pre-disconnected perforated Kirschner wire tension band in the treatment of patellar fracture in the elderly.Methods Seventy elderly patients with patellar fractures were divided in-to observation group(35 cases were treated with pre-disconnected perforated Kirschner wires tension band)and con-trol group(35 cases were treated with traditional AO Kirschner wires tension band),according to random number ta-ble method.The volume of intraoperative blood loss,operation time,the time to get out of bed for the first time,hospi-talization time,fracture healing time,postoperative complications,and knee joint activity were compared between the two groups.Knee joint function was evaluated according to Böstman scoring standard.Results All patients were fol-lowed up for 7～14 months.The volume of intraoperative blood loss,operation time and hospitalization time in the ob-servation group were less(shorter)than those in the control group(P＜0.05).The time to get out of bed for the first time and the fracture healing time had no statistical significance between the two groups(P＞0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P＜0.05).At 6 months after operation,the excellent and good rate of knee function evaluated by Böstman standard in the observation group was higher than that in the control group(P＜0.05).At the last follow-up,knee flexion rang of motion in the observa-tion group was greater than that in the control group(P＜0.05).Conclusions In the treatment of elderly patellar fracture,the pre-disconnected perforated Kirschner wire tension band has the advantages of less intraoperative blood loss,short operation time,short hospitalization time,fewer postoperative complications and good knee function recovery.
