双结克氏针张力带固定治疗髌骨骨折

摘要
目的 探讨双股钢丝"8"字捆绑克氏针张力带(即双结克氏针张力带)固定治疗髌骨骨折的效果.方法 采用双结克氏针张力带固定治疗50 例髌骨骨折患者.记录手术情况、骨折愈合时间、并发症发生情况.采用Lysholm评分标准评价膝关节功能.结果 患者均获得随访,时间6～15(8.6±5.2)个月.手术时间45～95(64.3±16.7)min,术中出血量10～45(26.5±9.6)ml,术后1d疼痛VAS评分2～6(3.3±1.0)分.骨折均完全愈合,时间10～16(12.8±3.0)周.术后发生内固定松动、退针2 例,创伤性关节炎1 例,软组织激惹2例,并发症发生率10%.术后6 个月,采用Lysholm评分标准评价膝关节功能:优 33 例,良 15 例,可 2 例,优良率96%.结论 双结克氏针张力带治疗髌骨骨折应力均匀分布,固定牢靠,内固定并发症少,疗效良好.
Abstract
Objective To investigate the effect of tension band fixation with double-strand wire 8-shaped binding Kir-schner wire(It's a double-knot Kirschner wire tension band)on the patellar fractures.Methods Fifty patients with patellar fractures were treated by tension band fixation with double-knot Kirschner wire tension band.The operation condition,fracture healing time and complications were recorded,and the knee joint function was evaluated by Lysholm score criterion.Results All patients were followed up for 6～15(8.6±5.2)months.The operation time was45～95(64.3±16.7)min,the intraoperative blood loss was 10～45(26.5±9.6)ml,and the pain VAS was 2～6(3.3±1.0)points at 1 d postoperation.All the fractures healed completely,and the healing time was 10～16(12.8±3.0)weeks.There were 2 case of internal fixation loosening and needle withdrawal,1 case of traumatic arthritis,and 2 ca-ses of soft tissue irritation,with a complication incidence rate of 10％.At6 months after operation,Lysholm score was used to evaluate the knee joint function:33 cases were excellent,15 good,and 2 fair,with an excellent-good rate of 96％.Conclusions The double-knot Kirschner wire tension band in the treatment of patellar fracture has the advan-tages of uniform distribution of stress,reliable fixation,few complications of internal fixation,with good effect.
