Treatment of ankle fractures with tibiotalar joint stabilization

摘要
目的 探讨胫距关节稳定术治疗踝关节骨折的疗效.方法 采用胫距关节稳定术治疗36 例踝关节骨折患者.记录术中出血量、手术时间、术后并发症发生情况、踝关节活动度.采用AOFAS踝-后足评分评价疗效.结果 患者均获得随访,时间2～9 个月.术中出血量 30～190(72.05±17.59)ml,手术时间 42～75(48.29±9.46)min.术后未发生距骨坏死、创伤性踝关节炎等并发症.骨折均愈合,时间 2.0～3.5 个月.末次随访时,踝关节活动度:背屈24°～27°,跖屈42°～47°;采用AOFAS踝-后足评分评价疗效:优23 例,良7 例,可6 例,优良率83.33%(30/36).结论 胫距关节稳定术治疗踝关节骨折操作简单,减少了手术创伤,减轻了患者的经济负担,疗效满意.
Abstract
Objective To investigate the effect of tibiotalar joint stabilization in the treatment of ankle fracture.Meth-ods Thirty-six cases of ankle fractures were treated with tibiotalar joint stabilization.The volume of intraoperative blood loss,operative time,postoperative complications and ankle range of motion were recorded.AOFAS ankle-hindfoot score was used to evaluate the efficacy.Results All patients were followed up for 2～9 months.The volume of intrao-perative blood loss was 30～190(72.05±17.59)ml,and the operative time was 42～75(48.29±9.46)min.No complications such as talus necrosis and traumatic ankle arthritis occurred after operation.All fractures healed within 2.0～3.5 months.At the last follow-up,the range of motion of ankle joint:dorsiflexion was 24°～27°,plantar flexion was 42°～47°;the AOFAS ankle-hindfoot score was used to evaluate the effect:excellent in23 cases,good in7,fair in 6,the excellent-good rate was 83.33%(30/36).Conclusions The operation of tibiotalar joint stabilization in the treatment of ankle fracture is simple,which reduce the surgical trauma and the economic burden of patients,the effect is satisfactory.
