摘要
目的：探讨腹茧症临床特点及诊治方法。方法回顾性分析2005年1月至2014年9月10例腹茧症患者的临床资料及随访结果。结果10例均表现为肠梗阻的症状，10例患者行腹部 X 线、6例 CT 检查均发现肠梗阻表现，3例 CT 发现腹腔内小肠聚集成团，可见包膜包裹，其余辅助检查未见明显异常。8例行剖腹术中均发现全部或部分小肠被包裹在一层坚韧的纤维膜内，行粘连松解、肠排列术等，7例手术病人全部治愈，另外2例经保守治疗后痊愈。结论患者反复出现肠梗阻症状，而又无其他原因解释及合并腹部包块者，应考虑腹茧症的可能，术前 CT 检查对本病的诊断很有价值，手术是主要的治疗方法，但术式选择需谨慎。
Abstract
Objective To explore the clinical characteristics and methods for diagnosis and treatment of patients with abdominal cocoon. Methods The clinical data of 10 patients with abdominal cocoon during January 2005 to September 2014 were retrospectively reviewed and ana-lyzed. Results All these 10 cases were manifested as symptoms of intestinal obstruction,and these 10 patients were examined with abdominal X- ray examination,CT scanning found performance of intestinal obstruction in six cases,three cases with small intestine gathered into groups formed amicula parcels,the remaining auxiliary examinations showed no abnormality. Laparotomy in 8 patients revealed that all or part of small in-testine had been wrapped in a layer of tough,fibrous membrane,and line adhesiolysis and other intestinal plication had been performed. Seven ca-ses were cured after surgical treatment,the other two cases recovered after conservative treatment. Conclusion If patients clinically manifested as symptoms of recurrent acute or chronic intestinal obstruction without other reasons or complicated with abdominal mass,abdominal cocoon should be considered. The preoperative CT scanning is very valuable in diagnosis of this disease. Operation is the main method for treatment,but the choice of procedure for operation needs to be cautious.
