Objective To investigate the expression of programmed cell death protein 1(PD-1)in hepatocellular carcinoma(HCC)mi-croenvironment cytotoxic T cells and its clinical significance.Methods A total of 344 HCC patients admitted to FuDan Univerisity Afflicated Zhongshan Hospital Qingpu Branch from January 2012 to December 2020 were retrospectively selected as the study subjects.The expression of PD-1 in the cancer tissues of surgically resected HCC patients was detected by immunohistochemical double staining,and divided into high PD-1 expression and low PD-1 expression according to the median expression level of PD-1.The correlation between PD-1 and clinicopathological features was analyzed.Kaplan-Meier method was used to calculate overall survival rate and progression-free survival rate,and survival curves were drawn.The prognostic factors of HCC was analyzed by using Multivariate Cox regression analysis.Results PD-1 was expressed on the cell membrane and/or cytoplasm of tumor infiltrating lymphocytes of HCC,and the high expression rate of PD-1 in tumor infiltrating lymphocytes was 43.9%(151/344).The positive expression rate of PD-L1 in HCC tumor group was 21.8%(75/344).The positive expression rate of PD-L1 in HCC tumor infiltrating immune cells was 47.1%(162/344).Tumor infiltrating CD8+T cell density was 45.3%(156/344).The high ex-pression of PD-1 was related to tumor histological grade,PD-L1 expression of tumor cells,PD-L1 expression of CD8+T lymphocytes,and CD8+T lymphocyte density(P<0.05).The overall survival rate of patients with high PD-1 expression was 80.81%,which was significantly lower than that of patients with low PD-1 expression(88.95%),and the difference was statistically significant(P<0.05);The disease-free survival rate of patients with high PD-1 expression was 67.44%,which was significantly lower than that of patients with low PD-1 expression(84.30%),and the difference was statistically significant(P<0.05).Multivariate Cox regression analysis showed that PD-L1 expression in tumor cells,PD-L1 expression in tumor infiltrating immune cells,and CD8+T lymphocyte density could be used as risk factors affecting the prog-nosis of patients with hepatocellular carcinoma(P<0.05).Conclusion CD8+T lymphocytes had a high expression rate of PD-L1,and the high expression of PD-1 in CD8+T cells was significantly correlated with tumor histological grade,PD-L1 expression in tumor cells,PD-L1 expression in CD8+T lymphocytes,CD8+T lymphocyte density,overall survival rate and disease-free survival rate.It can be used as one of the indicators to assist diagnosis and predict prognosis of HCC patients.
