Analysis of the effect of arthroscopic microfracture and the risk factors of postoperative lower extremity venous thrombosis in patients with traumatic knee osteoarthritis
Objective To analyze the effect of arthroscopic microfracture and the risk factors of postoperative lower extremity venous thrombosis in patients with traumatic knee osteoarthritis(TKOA).Methods A total of 80 patients with TKOA admitted to Jiamusi Central Hos-pital from March 2020 to March 2023 were retrospectively selected for study.They were divided into control group(n=30)and observation group(n=50)by different treatment methods.The control group underwent routine knee surgery,and the observation group underwent arthroscopic microfracture surgery.The clinical treatment effect and the incidence of lower extremity venous thrombosis in the two groups were analyzed.Ac-cording to whether lower extremity venous thrombosis ccoured after peration,the patients in the observation group were divided into thrombosis group and non-thrombosis group.The general characteristics of the two groups were analyzed,and the multivariate Logistic model was established to analyze the independent risk factors of lower extremity venous thrombosis after arthroscopic microfracture.Results The total effective rate of the observation group was 96.00%,which was higher than that of the control group(80.00%),and the difference was statistically significant(P<0.05).The incidence of lower limb venous thrombosis in the observation group was 24.00%,which was lower than that in the control group(46.67%),and the difference was statistically significant(P<0.05).Compared with the non-thrombus group,the age ≥60 years,hyper-lipidemia,tourniquet application time>90 min,postoperative bed time≥3 d,operation time ≥1 h and Caprini score were higher in the thrombus group(P<0.05).Univariate analysis showed that age,hyperlipidemia,tourniquet application time,postoperative bed rest time,operation time and Caprini score were the influencing factors of lower extremity venous thrombosis after arthroscopic microfracture in TKOA patients(P<0.05).Multivariate logistic analysis showed that age≥60 years,hyperlipidemia,tourniquet application time>90 min,postoperative bed rest time ≥3 d,operation time≥1 h,and Caprini score were all independent risk factors for lower extremity venous thrombosis after arthroscopic microfracture in patients with TKOA(P<0.05).Conclusion Arthroscopic microfracture is effective in the treatment of TKOA,but it is easy to cause lower ex-tremity venous thrombosis due to factors such as too old,postoperative bed rest time and too long operation time.Only by strengthening the preven-tion and intervention of such influencing factors can we effectively reduce the risk of lower extremity venous thrombosis in children.
