摘要

目的 探讨肺炎支原体(MPP)肺炎患儿外周血中白细胞介素(IL)-33和肿瘤发生抑制蛋白2(ST2)表达水平,并分析其与肺功能的相关性.方法 前瞻性选取2020年7月至2022年12月于黄石市妇幼保健院收治的96例2~10岁的MPP患儿,根据是否存在哮鸣音分为MPP喘息组(n=46)和MPP非喘息组(n=50),并选取同期健康儿童作为对照组(n=50).检测并比较3组研究对象血清中的IL-33、ST2水平和肺功能[呼气峰值流速(PEF)、用力呼气第一秒容积(FEV1)、FEV1/用力呼气第一秒容积占用力肺活量(FVC)的比例(FEV1/FVC)、以及在肺活量的75%、50%、25%水平时的最大呼气流速(V75、V50、V25)],分析MPP患儿外周血IL-33和ST2水平与肺功能指标的相关性.结果 MPP喘息组患儿外周血IL-33、ST2表达水平分别为(13.26±6.23)pg/mL、0.95±0.51,MPP非喘息组分别为(11.41±5.84)pg/mL、0.74±0.45,均明显高于对照组[(9.12±6.14)pg/mL、0.35±0.21],差异均有统计学意义(P<0.05).肺功能测试结果显示,MPP喘息组患儿PEF、FEV1、FEV1/FVC、V75、V50、V25水平分别为(72.61±5.81)%、(72.97±6.33)%、(73.84±5.79)%、(73.38±3.76)%、(43.38±8.87)%、(37.74±8.43)%,均明显低于 MPP 非喘息组[(90.62±6.62)%、(92.38±6.54)%、(84.57±4.47)%、(84.46±4.98)%、(78.83±6.77)%、(69.92±5.23)%],差异均有统计学意义(P<0.05).MPP患儿外周血IL-33和ST2的水平与PEF、FEV1、FEV1/FVC、V75、V50、V25均呈负相关.结论 MPP患儿的血清IL-33和ST2水平不仅与疾病的严重程度有关,还与肺功能的损害程度呈负相关,这为MPP的诊断和治疗提供了新的思路和依据.

Abstract

Objective To investigate the expression levels of interleukin33(IL-33)and tumorigenic suppressor protein 2(ST2)in pe-ripheral blood of children with mycoplasma pneumoniae(MPP)pneumonia,and to analyze their correlation with lung function.Methods A total of 96 children aged 2 to 10 years with MPP treated in Huangshi Maternal and Child Health Hospital admitted from July 2020 to December 2022 were prospectively selected and divided into the MPP wheezing group(n=46)and the MPP non-wheezing group(n=50)according to wheez-ing sound,and healthy children were selected as the control group(n=50).The serum levels of IL-33,ST2 and lung function[peak expiratory flow rate(PEF),forced expiratory volume in the first second(FEV1),ratio of FEV1/forced vital capacity(FVC)(FEV1/FVC),and maximum expiratory flow rate at 75%,50%,and 25%lung capacity levels(V75,V50,V25)]of the three groups were detected and compared,and the correlation between the levels of IL-33 and ST2 and lung function indexes in peripheral blood of MPP children was analyzed.Results The ex-pression levels of IL-33 and ST2 in peripheral blood of children with MPP wheezing were(13.26±6.23)pg/mL and 0.95±0.51,respective-ly,while those without MPP wheezing were(11.41±5.84)pg/mL and 0.74±0.45,respectively,which were significantly higher than those of the control group[(9.12±6.14)pg/mL and 0.35±0.21],and the differences were statistically significant(P<0.05).The pulmonary func-tion test results showed that the levels of PEF,FEV1,FEV1/FVC,V75,V50,and V25 in the MPP wheezing group were(72.61±5.81)%,(72.97±6.33)%,(73.84±5.79)%,(73.38±3.76)%,(43.38±8.87)%,and(37.74±8.43)%,respectively,which were signifi-cantly lower than those in the MPP non wheezing group[90.62±6.62)%,(92.38±6.54)%,(84.57±4.47)%,(84.46±4.98)%,(78.83±6.77)%,(69.92±5.23)%],the differences were statistically significant(P<0.05).Moreover,the levels of IL-33 and ST2 in peripheral blood of MPP children were negatively correlated with PEF,FEV1,FEV1/FVC,V75,V50,V25.Conclusion The serum levels of IL-33 and ST2 in children with MPP are not only related to the severity of the disease,but also negatively correlated with the degree of lung function damage,which provides a new idea and basis for the diagnosis and treatment of MPP.
