摘要

目的 研究Toll样受体(TLRs)/核因子(NF)-κB信号通路及肠道菌群特征与溃疡性结肠炎(UC)患者疾病活动度的关系.方法 前瞻性分析2021年1月至2023年1月山西白求恩医院收治的84例UC患者纳入本研究,设为观察组.将观察组患者根据Walmsley评分分为活动性溃疡性结肠炎(AUC)组(Walmsley评分>4分)45例和缓解性溃疡性结肠炎(RUC)组(Walmsley评分<4分)39例.另选同期发现为结肠息肉患者40例设为对照组.收集患者相关基线资料,比较3组患者肠道菌群分布、TLRs及NF-κB的相对表达量,分析TLRs及NF-κB的相对表达量与疾病活动度的相关性.结果 3组基线资料比较,差异均无统计学意义(P>0.05).AUC组、RUC组双歧杆菌数量分别为(7.06±0.15)、(6.25±0.31)lg CFU/g,均低于对照组[(8.45±0.53)lg CFU/g],大肠埃希菌数量分别为(8.95±0.24)、(10.12±0.43)lg CFU/g,均高于对照组[(7.46±0.47)lg CFU/g],差异均有统计学意义(P<0.05);RUC组乳杆菌数量为(5.04±0.12)lg CFU/g,低于对照组[(7.09±0.41)lg CFU/g],组间比较差异有统计学意义(P<0.05).AUC组患者 TLR2、TLR4、TLR5、TLR9 及 NF-κB 的 mRNA 表达量分别为 2.95±0.14、2.24±0.15、3.12±0.15、2.65±0.34、2.31±0.18,RUC 组 TLR2、TLR4、TLR5、TLR9 及 NF-κB 的 mRNA 表达量分别为 1.06±0.32、1.38±0.31、1.94±0.14、1.85±0.19、1.62±0.21,均高于对照组(1.01±0.21、1.01±0.21、0.98±0.06、1.03±0.08、0.94±0.03),AUC 组患者TLRs及NF-κB的mRNA表达量均高于RUC组,组间比较差异均有统计学意义(P<0.05).相关性分析显示,TLR2、TLR4、TLR5、TLR9 及 NF-κB 的相对表达量与 Walmsley 评分均呈正相关(r=0.458、0.384、0.556、0.641、0.674,P<0.05).结论 不同疾病活动度患者TLRs/NF-κB信号通路及肠道菌群分布存在差异,提示TLRs/NF-κB信号通路及肠道菌群可能影响UC患者疾病活动度.

Abstract

Objective To study the relationship between Toll-like receptors(TLRs)/nuclear factor(NF)-κB signaling pathway,in-testinal flora characteristics and disease activity in patients with ulcerative colitis(UC).Methods Prospective analysis of 84 UC patients admit-ted to Shanxi Bethune Hospital from January 2021 to January 2023 was included in this study and set as the observation group.The observation group patients were divided into the active ulcerative colitis(AUC)group(Walmsley score>4)with 45 cases and the remission ulcerative colitis(RUC)group(Walmsley score<4)with 39 cases based on the Walmsley score.Forty patients with colon polyps discovered during the same peri-od were selected as the control group.Patient related baseline data were collected and the distribution of gut microbiota,TLRs,and NF-κB a-mong three groups were compared,the correlation between the relative expression of TLRs and NF-κB and disease activity was analyzed.Results There was no statistically significant difference in baseline data among the three groups(P>0.05).The number of Bifidobacterium in the AUC group and the RUC group was(7.06±0.15)and(6.25±0.31)lg CFU/g,respectively,which was lower than that in the control group[(8.45±0.53)lg CFU/g],and the number of Escherichia coli was(8.95±0.24)and(10.12±0.43)lg CFU/g,respectively,which was higher than that in the control group[(7.46±0.47)lg CFU/g],the differences were statistically significant(P<0.05).The number of Lactobacillus in the RUC group was(5.04±0.12)lg CFU/g,which was lower than that in the control group[(7.09±0.41)lg CFU/g],and the differences were statistically significant(P<0.05).The mRNA expression levels of TLR2,TLR4,TLR5,TLR9 and NF-κB in the AUC group were 2.95±0.14,2.24±0.15,3.12±0.15,2.65±0.34 and 2.31±0.18,respectively,the mRNA expression levels of TLR2,TLR4,TLR5,TLR9 and NF-κB in the RUC group were 1.06±0.32,1.38±0.31,1.94±0.14,1.85±0.19 and 1.62±0.21,respectively,the mRNA expression levels of TLRs and NF-κB in the AUC group were higher than those in the RUC group(1.01±0.21,1.01±0.21,0.98±0.06,1.03±0.08,0.94±0.03),and the differences were statistically significant(P<0.05).Correlation analysis showed that the relative expression levels of TLR2,TLR4,TLR5,TLR9 and NF-κB were positively correlated with Walmsley score(r=0.458,0.384,0.556,0.641,0.674;P<0.05).Conclusion There are differences in TLRs/NF-κB signaling pathway and intestinal flora distribution in patients with differ-ent disease activity,suggesting that TLRs/NF-κB signaling pathway and intestinal flora may affect the disease activity of UC patients.
