外周血微RNA-497a、淀粉样前体蛋白β位点裂解酶1对妊娠期肥胖孕妇的预测价值分析

摘要
目的 探讨外周血微RNA(miRNA)-497a、淀粉样前体蛋白β位点裂解酶1(BACE1)对妊娠期肥胖的预测价值.方法 前瞻性选取2020年4月至2022年4月南通大学附属南通市妇幼保健院收治的100例妊娠期肥胖孕妇作为观察组.参照2001年中国肥胖问题工作组制定的中国成人体重指数(BMI)分类建议将其分为孕前肥胖组(n=26,孕前BMI≥28 kg/m2,孕期体重增加<20 kg)、孕期肥胖组(n=42,孕前BMI 18~23.9 kg/m2,孕期体重增加≥20 kg)和孕前+孕期肥胖(n=32,孕前BMI≥28 kg/m2,孕期体重增加≥20 kg);另选取正常健康孕妇40例作为正常体重组(孕前BMI 18~23.9 kg/m2,孕期体重增加<20 kg).采用荧光定量PCR(qRT-PCR)检测各组的外周血miR-497a水平,另采用酶联免疫吸附试验法检测外周血BACE1水平.比较各组及妊娠期肥胖孕妇不同临床特征的外周血miR-497a、BACE1水平;采用双变量Spearman相关性分析对外周血miR-497a、BACE1与妊娠期肥胖孕妇的相关性进行分析;建立多因素Logistic回归模型分析影响妊娠期肥胖孕妇的独立危险因素,并受试者操作特征(ROC)曲线评估外周血miR-497a、BACE1对妊娠期肥胖的预测价值.结果 与正常体重组比较,孕前肥胖组、孕期肥胖组、孕前+孕期肥胖组外周血miR-497a、BACE1水平均更高,差异均有统计学意义(P<0.05).不同年龄、孕周、孕次妊娠期肥胖孕妇的外周血miR-497a、BACE1水平比较,差异均无统计学意义(P>0.05);BMI≥30 kg/m2妊娠期肥胖孕妇的外周血miR-497a、BACE1水平均明显高于BMI<30 kg/m2,差异均有统计学意义(P<0.05).Spearman相关性分析结果显示,外周血miR-497a、BACE1与妊娠期肥胖均呈正相关性(r=0.787、0.758,P<0.05).多因素Logistic回归分析结果显示,miR-497a、BACE1均是引发妊娠期肥胖的独立危险因素(P<0.05).miR-497a+BACE1在妊娠期肥胖预测中的特异度明显高于miR-497a、BACE1单独检测,曲线下面积(AUC)值达0.737.结论 外周血miR-497a、BACE1与妊娠期肥胖均存在一定关联,根据其水平变化情况有利于预测妊娠期肥胖的发生风险.
Abstract
Objective To study the predictive value of microRNA(miRNA)-497a and beta-site amyloid precursor protein clearing en-zyme-1(BACE1)in peripheral blood for obesity during pregnancy.Methods A total of 100 obese pregnant women admitted to Affiliated Ma-ternity and Child Health Care Hospital of Nantong University from April 2020 to April 2022 were prospectively selected as the observation group.They were divided into the pre-pregnancy obesity group(n=26,pre-pregnancy BMI ≥28 kg/m2,weight gain during pregnancy<20 kg),the pregnancy obesity group(n=42,pre-pregnancy BMI 18-23.9 kg/m2,weight gain during pregnancy ≥ 20 kg)and the pre-pregnancy+pregnancy obesity group(n=32,pre-pregnancy BMI ≥28 kg/m2,weight gain during pregnancy ≥20 kg)according to the recommendation of Chinese adult body mass index(BMI)classification developed by China Obesity Working Group in 2001.In addition,40 normal healthy pregnant women were selected as the normal weight group(pre-pregnancy BMI 18-23.9 kg/m2,weight gain during pregnancy<20 kg).Quantitative re-al-time(qRT)-PCR was used to detect the level of miR-497a in peripheral blood of each group,and the level of BACE1 in peripheral blood was detected by enzyme-linked immunosorbent assay.The levels of miR-497 a and BACE1 in peripheral blood of each group and pregnant women with different clinical characteristics were compared.The correlation between peripheral blood miR-497a,BACE1 and obese pregnant women during pregnancy was tested by bivariate Spearman correlation annalysis.Multivariate Logistic regression model was established to analyze the independent risk factors affecting pregnant women with obesity during pregnancy,and to analyze its predictive value.Results Compared with the normal weight group,the levels of miR-497a and BACE1 in peripheral blood of pre-pregnancy obesity group,pregnancy obesity group and pre-pregnancy+pregnancy obesity group were higher,and the differences were statistically significant(P<0.05).There were no statistically significant difference in the levels of miR-497a and BACE1 in peripheral blood of obese pregnant women with different ages,gestational weeks and gestational times(P>0.05).The levels of miR-497a and BACE1 in peripheral blood of obese pregnant women with BMI ≥30 kg/m2 were significantly higher than those with BMI<30 kg/m2,and the differences were statistically significant(P<0.05).Peripheral blood miR-497a and BACE1 were positively correlated with gestational obesity(r=0.787,0.758;P<0.05).Multivariate Logistic regression analysis showed that miR-497 a and BACE1 were independent risk factors for gestational obesity(P<0.05).The specificity of miR-497a+BACE1 in the prediction of gestational obesity was significantly higher than that of miR-497a and BACE1 alone,and the area under the curve(AUC)value was 0.737.Conclusion Peripheral blood miR-497 a and BACE1 are associated with gestational obesity,and the changes of their levels are helpful to predict the risk of gestational obesity.
