摘要
目的 探讨不同类型动脉瘤致蛛网膜下腔出血CT分布差异.方法 回顾性选取2019年8月至2023年12月在宣城市人民医院接受治疗的动脉瘤致蛛网膜下腔出血患者100例,均给予CT检查,分析患者责任动脉瘤及出血分布情况,比较不同预后患者临床资料差异.结果 共检出责任动脉瘤100枚,包括Ⅰ类动脉瘤78枚,Ⅱ类动脉瘤22枚,Ⅰ类动脉瘤分布以颈内动脉(ICA)、前交通动脉(ACoA)为主,达到89.74%,Ⅱ类动脉瘤分布以大脑中动脉(MCA)M1段为主.Ⅰ类动脉瘤出血位于鞍上池、环池比例分别为96.15%和92.31%,明显高于Ⅱ类动脉瘤(72.73%、68.18%),差异均有统计学意义(P<0.05);ACoA、ICA和后交通动脉(PCoA)动脉瘤出血位于鞍上池、环池、侧裂池、纵裂、四叠体池和桥前池比较,差异均无统计学意义(P>0.05);MCA M1和M2-4段A动脉瘤出血位于鞍上池、环池、侧裂池、纵裂、四叠体池和桥前池比较,差异均无统计学意义(P>0.05).结论 在动脉瘤致蛛网膜下腔出血中,不同类型位动脉瘤出血CT分布存在差异,Ⅰ类动脉瘤出血位于鞍上池、环池比例高于Ⅱ类动脉瘤.
Abstract
Objective To explore the differences in CT distribution of subarachnoid hemorrhage caused by aneurysms in different types.Methods One hundred patients with subarachnoid hemorrhage caused by aneurysms who were treated in Xuancheng Peoples Hospital from August 2019 to December 2023 were retrospectively selected,all of whom underwent CT examinations to analyze the distribution of responsible aneurysms and bleeding,and to compare the differences in clinical data among patients with different prognosis.Results A total of 100 responsible aneu-rysms were detected,including 78 class Ⅰ aneurysms and 22 class Ⅱ aneurysms.Class Ⅰ aneurysms were mainly distributed in the internal ca-rotid artery(ICA)and anterior communicating artery(ACoA),accounting for 89.74%.Class Ⅱ aneurysms were mainly distributed in the M1 segment of the middle cerebral artery(MCA).The proportions of class Ⅰ aneurysms with bleeding located in the suprasellar cistern and annular cistern were 96.15%and 92.31%,respectively,which were significantly higher than those of class Ⅱ aneurysms(72.73%,68.18%),the differences were statistically significant(P<0.05).There were no statistically significant differences between ACoA,ICA and posterior commu-nicating artery(PCoA)aneurysms with bleeding located in the suprasellar cistern,annular cistern,lateral fissure cistern,longitudinal fissure,quadrifold cistern,and anterior pontine cistern(P>0.05).There were no statistically significant differences in the location of hemorrhage from MCA M1 and M2-4 segment A aneurysms in the suprasellar cistern,annular cistern,lateral fissure cistern,longitudinal fissure,quadrilateral cis-tern,and anterior pontine cistern(P>0.05).Conclusion In subarachnoid hemorrhage caused by aneurysms,there are differences in CT distri-bution of aneurysmal bleeding in different types,the proportion of bleeding from class Ⅰ aneurysms located in the suprasellar cistern and annular cistern is higher than that of class Ⅱ aneurysms.
