荜铃胃痛颗粒联合富马酸伏诺拉生片治疗胃食管反流病的效果及对患者症状、胃肠分泌功能的影响

摘要
目的 研究荜铃胃痛颗粒联合富马酸伏诺拉生片治疗胃食管反流病(GERD)的临床效果,并探讨其治疗对患者的临床症状和胃肠分泌功能的影响.方法 回顾性选取2021年1月至2023年12月在延安市中医医院接受治疗的GERD患者112例,根据临床治疗方案不同分为对照组(n=56)和研究组(n=56).对照组患者给予富马酸伏诺拉生片治疗,研究组患者给予荜铃胃痛颗粒联合富马酸伏诺拉生片治疗,两组患者均治疗8周.比较两组患者临床疗效,治疗前、治疗8周后的中医症候积分、GERD问卷评分、DeMeester评分,以及血清胃动素、胃泌素和血管活性肠肽(VIP)水平,并观察两组不良反应发生情况.结果 研究组治疗后临床治疗总有效率为96.43％,显著高于对照组(83.93％),差异有统计学意义(P＜0.05).治疗8周后,两组患者的中医症候积分均较治疗前降低,且研究组胃灼热、反酸中医症候积分分别为(1.42±0.61)、(1.63±0.54)分,均低于对照组[(2.05±0.73)、(2.11±0.63)分],差异均有统计学意义(P＜0.05).治疗8周后,两组GERD问卷评分、DeMeester评分均较治疗前降低,且研究组GERD问卷评分、DeMeester 评分分别为(7.89±1.37)、(23.65±7.45)分,均显著低于对照组[(11.65±1.92)、(28.97±7.65)分],差异均有统计学意义(P＜0.05).治疗8周后,两组血清胃动素和胃泌素水平均较治疗前升高,血清VIP水平均较治疗前下降,且研究组的血清胃动素、胃泌素水平分别为(260.58±22.17)、(67.69±4.58)pg/mL,均高于对照组[(248.35±21.63)、(58.93±5.51)pg/mL],VIP 水平为(17.35±2.72)pg/mL,低于对照组[(20.53±2.89)pg/mL],差异均有统计学意义(P＜0.05).研究组治疗期间总不良反应发生率与对照组患者比较(19.64％vs.16.07％),差异无统计学意义(P＞0.05).结论 荜铃胃痛颗粒联合富马酸伏诺拉生片治疗GERD临床疗效较好,可显著改善患者中医症候积分和临床症状,提高患者胃肠道激素分泌水平.
Abstract
Objective To study the clinical effect of Long Bell stomach pain granules combined with vonora fumarate raw tablets in the treatment of gastroesophageal reflux disease(GERD),and explore the effects of clinical symptoms and gastrointestinal hormones in the treatment of patients.Methods A total of 112 patients with GERD who received treatment in Yan'an Traditional Chinese Medicine Hospital from January 2021 to December 2023 were retrospectively selected,and were divided into the control group(n=56)and the study group(n=56)according to dif-ferent clinical treatment regimens.The patients in the control group were treated with vonora fumarate raw tablets,and the patients in the study group were treated with Long Bell stomach pain granules combined with vonora fumarate raw tablets,both groups were treatment for 8 weeks.The clinical efficacy,traditional chinese medicine symptom score,GERD questionnaire score,DeMeester score,and serum motilin,gastrin and vaso-active intestinal peptide(VIP)levels before treatment and after 8 weeks of treatment were compared between the two groups,and the occurrence of adverse reactions in the two groups were observed.Results The total clinical response rate of the study group was 96.43％,which was higher than that of the control group(83.93％),the difference was statistically significant(P＜0.05).After 8 weeks of treatment,the scores of tradi-tional chinese medicine symptoms in the two groups were lower than those before treatment,and the scores of traditional chinese medicine symptoms of heartburn and acid reflux in the study group were(1.42±0.61)and(1.63±0.54)points,respectively,which were lower than those in the control group[(2.05±0.73),(2.11±0.63)points],the differences were statistically significant(P＜0.05).After 8 weeks of treatment,the scores of GERD questionnaire and DeMeester were lower than those before treatment,and the scores of GERD questionnaire and DeMeester in the study group were(7.89±1.37)and(23.65±7.45)points,respectively,which were significantly lower than those in the control group[(11.65±1.92)and(28.97±7.65)points],the differences were statistically significant(P＜0.05).After 8 weeks of treatment,the levels of serum motilin and gastrin in the two groups were higher than those before treatment,and the levels of serum VIP were lower than those before treatment.The levels of serum motilin and gastrin in the study group were(260.58±22.17)and(67.69±4.58)μg/mL,respectively,which were higher than those in the control group[(248.35±21.63)and(58.93±5.51)pg/mL],and the level of VIP was(17.35±2.72)μg/mL,which was lower than that in the control group[(20.53±2.89)pg/mL],the differences were statistically significant(P＜0.05).There was no statistically significant difference in the total incidence of adverse reactions between the study group and the control group during treatment(19.64％vs.16.07％)(P＞0.05).Conclusion The combination of Long Bell stomach pain granules and vonora fumarate raw tablets in the treatment of GERD has good clinical efficacy,which can significantly improve the traditional chinese medicine syndrome score and clinical symp-toms of patients,and improve the gastrointestinal secretion function of patients.
