摘要
目的 研究可溶性CD86(sCD86)、高迁移率族蛋白B1(HMGB1)水平与过敏性鼻炎患儿病情的相关性.方法 回顾性选取2020年～2024年邯郸市妇幼保健院诊疗的100例过敏性鼻炎患儿,列入观察组,按照疾病严重程度分为轻度组(n=52)和中重度组(n=48);并选取同期入院体检的健康儿童100名,列入对照组.分析并对比观察组与对照组儿童、轻度组与中重度组患儿的血清sCD86、HMGB1水平以及淋巴细胞亚群(CD3+、CD4+、CD8+)和血清IgE水平,并对观察组患儿血清sCD86、HMGB1水平与淋巴细胞亚群和IgE水平的相关性进行分析.结果 观察组患儿的sCD86 和 HMGB1 水平分别为(0.59±0.12)ng/mL、(249.73±40.52)μg/mL,均明显高于对照组[(0.41±0.03)ng/mL、(61.84±10.33)μg/mL],CD3+、CD4+、CD8+水平分别为(63.12±2.37)％、(35.15±3.12)％、(23.52±2.25)％,均明显低于对照组[(68.59±3.23)％、(41.22±3.48)％、(28.67±2.34)％],血清 IgE 水平为(360.83±100.58)IU/mL,明显高于对照组[(35.27±10.64)IU/mL],差异均有统计学意义(P＜0.05).中重度组患儿血清sCD86和HMGB1水平分别为(0.62±0.04)ng/mL、(257.43±4.32)μg/mL,均明显高于轻度组[(0.51±0.08)ng/mL、(232.89±4.71)μg/mL],中重度组 CD3+、CD4+、CD8+水平分别为(61.22±3.46)％、(34.23±2.44)％、(22.51±2.28)％,均明显低于轻度组[(65.14±3.69)％、(38.58±2.51)％、(25.32±2.18)％],血清 IgE 水平为(317.66±57.43)IU/mL,明显高于轻度组[(248.53±52.64)IU/mL],差异均有统计学意义(P＜0.05).观察组患儿的血清sCD86、HMGB1水平与CD3+、CD4+、CD8+水平均呈负相关(P＜0.05),与血清IgE水平呈正相关(P＜0.05).结论 过敏性鼻炎患儿血清sCD86、HMGB1水平呈现高表达,且随病情加重而增高,这可能与Th1和Th2这两种辅助性T细胞亚群的平衡失调有关.
Abstract
Objective To study the correlation between soluble CD86(sCD86)and high mobility group box l(HMGB1)levels and the condition of children with allergic rhinitis.Methods A retrospective selection was performed for 100 children with allergic rhinitis who were trea-ted in Handan Maternal and Child Health Hospital from 2020 to 2024,all the children included in the observation group.They were divided into the mild group(n=52)and the moderate to severe group(n=48)according to the severity of the disease.And 100 healthy children admitted for physical examination during the same period were included in the control group.The levels of serum sCD86 and HMGB1,lymphocyte subsets(CD3+,CD4+,CD8+)and serum IgE in the the observation group and the control group,the mild group and the moderate to severe group were analyzed and compared.And the correlation between serum sCD86 and HMGB1 levels and lymphocyte subsets and IgE levels in the observation group was analyzed.Results The levels of sCD86 and HMGB1 in the observation group were(0.59±0.12)ng/mL and(249.73±40.52)μg/mL,respectively,which were significantly higher than those in the control group[(0.41±0.03)ng/mL,(61.84±10.33)μg/mL],the levels of CD3+,CD4+and CD8+were(63.12±2.37)％,(35.15±3.12)％and(23.52±2.25)％,respectively,which were significantly lower than those in the control group[(68.59±3.23)％,(41.22±3.48)％,(28.67±2.34)％],and the serum IgE level was(360.83±100.58)IU/mL,which was significantly higher than that in the control group[(35.27±10.64)IU/mL],the differences were statistically sig-nificant(P＜0.05).The levels of serum sCD86 and HMGB1 in the moderate to severe group were(0.62±0.04)ng/mL and(257.43±4.32)μg/mL,respectively,which were significantly higher than those in the mild group[(0.51±0.08)ng/mL,(232.89±4.71)μg/mL],the lev-els of CD3+,CD4+and CD8+in the moderate to severe group were(61.22±3.46)％,(34.23±2.44)％and(22.51±2.28)％,respec-tively which were significantly lower than those in the mild group[(65.14±3.69)％,(38.58±2.51)％,(25.32±2.18)％],and the ser-um IgE level was(317.66±57.43)IU/mL,which was significantly higher than that in the mild group[(248.53±52.64)IU/mL],the differ-ences were statistically significant(P＜0.05).The levels of serum sCD86 and HMGB1 in the observation group were negatively correlated with the levels of CD3+,CD4+and CD8+(P＜0.05),and positively correlated with the level of serum IgE(P＜0.05).Conclusion Children with allergic rhinitis,sCD86,HMGB1 levels present high expression,which may be related to Th1 and Th2 balance of these two kinds of helper T cells subgroup associated disorders.
