首发与复发急性期精神分裂症患者静息态低频振幅与分数低频振幅分析

目的:分析首发和复发急性期精神分裂症患者静息态低频振幅与分数低频振幅改变情况.方法:纳入2014 年6 月至2020 年05 月在武汉大学人民医院精神卫生中心60 例首发、60 例复发急性期精神分裂症患者及60 名健康对照者进行静息态功能磁共振扫描,采用基于体素水平的低频振幅(ALFF)及分数低频振幅(fALFF)指标分析3 组间局部脑功能活动情况.结果:协方差分析显示三组间ALFF值存在显著差异的脑区位于双侧纹状体(左侧F = 23.06,P<0.001;右侧F = 19.31,P<0.001),而fALFF值存在显著差异的脑区位于双侧纹状体(左侧F =19.71,P<0.001;右侧F =13.53,P<0.001)和前额叶内侧皮质(F =5.46,P<0.001);事后检验显示与对照组相比,首发组和复发组双侧纹状体ALFF及fALFF值均显著升高、内侧前额叶皮质fALFF值降低;复发组内侧前额叶皮质fALFF值降低较首发组更为明显.结论:首发和复发急性期精神分裂症患者均存在双侧纹状体、内侧前额叶皮质的局部功能活动异常,而复发精神分裂症患者内侧前额叶皮质局部脑功能损害更显著.
Resting state study of amplitude of low-frequency fluctuation and fractional amplitude of low-frequency fluctuation in acute-phase patients with first-episode and multi-episode schizophrenia
Objective:The purpose of this study was to investigate local activity differences in first-epi-sode and multi-episode acute schizophrenia patients using amplitude of low-frequency fluctuation(ALFF)and fractional amplitude of low-frequency fluctuation(fALFF)based on resting-state.Method:Sixty patients with first-episode schizophrenia(FES group),sixty patients with multi-episode schizophrenia(MES group),and sixty healthy controls(HC group)in Department of Psychiatry,Renmin Hospital of Wuhan University from June 2014 to May 2020 were recruited for the study and underwent resting-state fMRI.The individual voxel-wise ALFF and fALFF maps were obtained and used to compare differences in the FES and MES groups with the HC group.Results:Significant ALFF differences were found in the bilateral striatum,while significant fALFF differences were found in the bilateral striatum and medial prefrontal cortex(MPFC)across all three groups.Compared with the HC group,both the FES and MES groups showed increased ALFF and fALFF in the bilateral striatum and decreased fALFF in the MPFC.In addition,the MES patients exhibited reduced fALFF in the MPFC compared to the FES group.Conclusion:The result suggests that both first-episode and multi-episode acute schizophrenia patients exhibit abnormal local neural functional activities in the striatum and MPFC.Fur-thermore,aberrant local activity of the MPFC tends to be more pronounced in the MES group.
