河南省医学院校学生抑郁现状及影响因素的性别差异研究

目的:调查医学生群体的抑郁现状,并进一步探讨抑郁相关的临床特征和风险因素的性别差异.方法:对某医学院校采取整群抽样的方式进行心理健康筛查,共获有效样本 7657 例.使用卡方检验和双因素方差分析比较性别间差异,二元逻辑回归分析抑郁比值比(OR).结果:男性和女性的抑郁症状检出率分别为 26.2%和25.0%.吸烟、饮酒和压力感知异常在男性中更常见,而女性更愿意接受心理干预.抑郁主要影响因素为饮酒、心理干预意愿、焦虑和失眠.男性的独立保护因素为近一年未吸烟(OR=0.350);其独立风险因素为精神疾病家族史(OR=3.150).女性的独立保护因素为年龄增长(OR=0.918);其独立风险因素为自杀意念(OR=1.224).结论:医学生抑郁症状的检出率较高,应进行抑郁症状的早期识别,并根据性别制定个性化干预措施,提升心理健康水平.
Gender difference of depression status and influencing factors among medical students
Objective:To investigate the depression status of medical students and explore its influencing factors according to gender.Method:A cluster sampling method was adopted for mental health screening at a certain medical college in Henan and valid data from 7657 students were analysed.Chi-square tests and two-way ANOVA were used to compare differences between male and female students,and binary logistic regression analysis was applied to calculate the odds ratio(OR)of depression.Results:The detection rates of depressive symptoms in male and female medical students were 26.2%and 25.0%,respectively.Smoking,alcohol con-sumption,and abnormal stress perception were more common in males,while females were more willing to accept psychological interventions.Major factors influencing depression included alcohol consumption,willingness for psychological intervention,anxiety,and insomnia.In males,not smoking in the past year was a protective factor(OR=0.350),and a family history of mental illness was a risk factor(OR=3.150).In females,increasing age was protective(OR=0.918),and suicidal ideation was a risk factor(OR=1.224).Conclusion:High detection rates of depressive symptoms were observed among medical students.Early identification and personalized intervention measures based on gender are necessary to improve the psychological health of medical students.
