摘要
目的:了解老年抑郁症(late life depression,LLD)患者认知功能的影响因素.方法:选取伴有认知障碍的LLD患者118例,采用一般资料调查表、汉密尔顿抑郁量表17项(Hamilton depression scale 17 items,HAMD-17)及阿尔茨海默病评定量表-认知分量表(Alzheimer's disease assessment scale-cognitive section,ADAS-cog)调查伴有认知障碍的LLD患者的基本情况、抑郁症状及认知功能.采用分析方法,探讨对伴有认知障碍的LLD患者认知功能的影响因素.结果:单因素分析显示有吸烟史、有饮酒史、有家族史、抑郁发作次数增加、既往无抽搐电休克治疗(modified electroconvulsive therapy,MECT)次数增加、收缩压升高、体质量指数(body mass index,BMI)升高均能增加ADAS-cog的分值.多元线性回归分析显示有吸烟史、有饮酒史、有精神疾病家族史、抑郁发作次数增加、HAMD-17数值升高、BMI数值升高、收缩压升高均能增加ADAS-cog的分值,偏回归系数(β)均>0.结论:抑郁症的发作次数、抑郁症状严重程度、血压值、BMI及吸烟饮酒史均是老年抑郁症患者认知功能障碍的独立危险因素.
Abstract
Objective:To understand the influencing factors of cognitive function in patients with late life depression(LLD).Method:A total of 118 LLD patients with cognitive impairment were selected,and their basic information,depressive symptoms,and cognitive function were investigated using the general information survey,Hamilton depression scale 17 items(HAMD-17),and Alzheimer's disease assessment scale cognitive section(ADAS-cog).The influencing factors of cognitive function in LLD patients with cognitive impairment were explored by analysis methods.Results:Univariate analysis showed that a history of smoking,alcohol consumption,family history,increased frequency of depressive episodes,increased frequency of modified electroconvulsive therapy(MECT),elevated systolic blood pressure,and elevated body mass index(BMI)all increased ADAS-cog scores.Multiple linear regression analysis showed that having a history of smoking,drinking,family history of mental illness,increased frequency of depressive episodes,elevated HAMD-17 values,elevated BMI values,and elevated systolic blood pressure all increased ADAS-cog scores,with all partial regression coefficients(β)>0.Conclusion:The frequency of depressive episodes,severity of depressive symptoms,blood pressure,BMI,and smoking and drinking history are all independent risk factors for cognitive function in LLD with cognitive impairment.
