Features of actinic keratosis under dermatoscopy and reflection confocal microscopy
Objective:To investigate polarized light dermoscopic and reflectance confocal miscroscopic features of actinic ker-atosis(AK).Methods:Thirty-five patients with 45 lesions of AK were collected in Dermatology Hospital of Southern Medical University from January 2021 to October 2023,and the dermoscopic and miscroscopic features were analyzed.Results:Among the 35 patients,including 16 males and 19 females,30 cases showed non-pigmented AK and 5 cases were pigmented AK,with a total of 45 lesions.Polarized light dermoscopy showed yellowish-white scales,red or brown perifollicle pseudonetwork,perifollicle white circles and follicular keratotic plugs,rosettes sign,vascular structure and gray granules in red and reddish-brown back-ground.Reflectance confocal miscroscopy(RCM)examination showed hyperkeratosis and parakeratosis,disorganized prickle cells,irregular target cells in the epidermis,tortuous blood vessels with inflammation cells,dendritic cells and pigmentophage in the superficial dermis and elastic degeneration of dermal collagen fibers.Red background and red pseudonetwork were mainly ob-served in non-pigmented AK under dermoscopy.Conversely,reddish-brown background,brown pseudonetwork and gray gran-ules mainly showed in pigmented AK.Pigmentophages were mainly observed in pigmented AK by RCM,while other features were similar between non-pigmented and pigmented AK.Conclusion:AK displays specific characteristics under polarized light dermoscopy and RCM,providing a basis for non-invasive diagnosis of the disease.
