A case of granulomatous slack skin is reported.A 46-year-old male presented with generalized flaccid skin plaques for 5 years.Dermatological examination showed multiple soft-textured plaques on the neck,both upper limbs,both axillae,both groins,chest and abdomen,and lumbar back,with wrinkled surface skin lines and skin laxity.Histopathological examination of the lesions on the left upper limb revealed multifocal lymphoid cells scattered under the deep dermis and subcutaneous fat tis-sue,and histiocyte infiltration with the formation of multinucleated giant cells.Multinucleated giant cells infiltrated the dermis,and were arranged in a ring or aggregated on one side of the cells,surrounded by a few lymphoid cells.Immunohistochemical staining showed that lymphoid cells were positive for CD3,CD4,CD5,and slightly positive for CD7 and CD8.The cell prolif-eration marker Ki-67 was positive(<5%);the cells were negative for CD20,CD30,CD56 and granzyme B(Gra B).A diagnosis of granulomatous slack skin was made.
