摘要
目的 探讨以非睾丸疼痛为首发症状的睾丸扭转临床特点及误诊原因、防范措施.方法 回顾性分析2015 年1 月—2022 年10 月收治的曾误诊的以非睾丸疼痛为首发症状睾丸扭转3 例的临床资料.结果 本文3 例分别因下腹部疼痛伴恶心、呕吐15 h,左侧腹股沟区疼痛伴排尿困难2h及脐周疼痛伴恶心、渐出现右下腹部疼痛 30h就诊.3 例分别误诊为急性胃肠炎、腹股沟嵌顿疝和急性阑尾炎.术中探查明确睾丸扭转诊断后 2 例行患侧睾丸切除加对侧睾丸固定术,1 例行双侧睾丸固定术.结论 临床医生要警惕以非睾丸疼痛为首发症状的睾丸扭转,重视查体,并配合进行阴囊彩色多普勒超声检查,必要时尽快手术探查,以减少或避免睾丸扭转误诊造成不良预后.
Abstract
Objective To investigate the clinical characteristics,causes of misdiagnosis and preventive measures of testicular torsion with non-testicular pain as the initial symptom.Methods The clinical data of 3 patients with testicular tor-sion with non-testicular pain as the initial symptom from January 2015 to October 2022 were analyzed retrospectively.Results In this study,3 patients presented with lower abdominal pain accompanied by nausea and vomiting for 15 h,left groin pain with dysuria for 2 h,and periumbilical pain with nausea and progressive right lower abdominal pain for 30 h,respectively.Three cases were misdiagnosed as acute gastroenteritis,incarcerated inguinal hernia and acute appendicitis,respectively.Af-ter the diagnosis of testicular torsion was confirmed by intraoperative exploration,2 patients underwent testicectomy and contra-lateral testicopexy,and 1 patients underwent bilateral testicopexy.Conclusion Clinicians should be alert to testicular torsion with non-testicular pain as the initial symptom,pay attention to physical examination,perform scrotal color Doppler ultrasonog-raphy and surgical exploration as soon as necessary,in order to reduce or avoid misdiagnosis of testicular torsion,which may result in poor prognosis.
