摘要
目的 探究肠内营养支持治疗对原发性肝癌肝动脉化疗栓塞(TACE)术后患者胆碱酯酶(CHE)、白蛋白(Alb)、二胺氧化酶(DAO)及肝功能、免疫功能的影响.方法 选取 2019 年 1 月—2022 年 3 月收治的 119 例原发性肝癌,按照营养支持方法的不同分为试验组60 例(予肠内营养支持)和对照组59 例(予肠外营养支持).比较2 组治疗前后CHE、Alb、DAO及肝功能、免疫功能指标水平和不良反应发生情况.结果 治疗后,2 组CHE、Alb、DAO水平均上升,且试验组高于对照组(P＜0.05,P＜0.01).治疗后,2 组天冬氨酸转氨酶、丙氨酸转氨酶、间接胆红素水平均下降,且试验组低于对照组(P＜0.05,P＜0.01).治疗后,2 组CD4+、CD4/CD8+水平升高,CD8+水平下降,且试验组CD4+、CD4+/CD8+水平高于对照组,CD8+水平低于对照组(P＜0.05,P＜0.01).试验组腹泻、腹胀、呕吐不良反应总发生率低于对照组(P＜0.01).结论 肠内营养支持治疗能改善原发性肝癌TACE术后患者CHE、Alb、DAO及肝功能、免疫功能,且不良反应少,安全性高.
Abstract
Objective To investigate the effects of enteral nutrition support therapy on cholinesterase(CHE),albu-min(Alb),diamine oxidase(DAO),liver function and immune function in patients with primary liver cancer after hepatic arterial chemoembolization(TACE).Methods In total,119 patients with primary liver cancer treated from January 2019 to March 2022 were selected and divided into experimental group(receiving enteral nutrition support,n =60)and control group(receiving parenteral nutrition support,n =59)according to different nutritional support methods.CHE,Alb,DAO,liver function,immune function indexes and the occurrence of adverse reactions were compared between the two groups before and after treatment.Results After treatment,CHE,Alb and DAO levels in the two groups were increased,which were higher in the experimental group than in the control group(P＜0.05,P＜0.01).After treatment,the levels of aspartate aminotrans-ferase,alanine aminotransferase and indirect bilirubin in the two groups were decreased,which were lower in the experimental group than in the control group(P＜0.05,P＜0.01).After treatment,the levels of CD4+ and CD4+/CD8+ were in-creased and CD8+ was decreased in the two groups,and the levels of CD4+ and CD4+/CD8+ in experimental group were higher than those in control group,while the levels of CD8+ were lower than those in control group(P＜0.05,P＜0.01).The total incidence of adverse reactions,including diarrhea,abdominal distension and vomiting in the experimental group was lower than that in control group(P＜0.01).Conclusion Enteral nutrition support therapy can improve CHE,Alb,DAO,liver function and immune function of patients with primary liver cancer after TACE,and has fewer adverse reactions and bet-ter safety.
