甲状腺癌患者NF-κB mRNA、miR-125a-5p与临床病理特征及预后的关系

摘要
目的 探究甲状腺癌患者核转录因子-κB(NF-κB)mRNA、miR-125a-5p与临床病理特征及预后的关系.方法 选取2017 年5 月—2020 年4 月240 例甲状腺癌手术患者作为观察组,另选取同期体检健康者60 例作为对照组.比较2 组NF-κB mRNA、miR-125a-5p水平,分析甲状腺癌患者NF-κB mRNA、miR-125a-5p水平与临床病理特征的关系,比较复发与未复发患者临床资料,分析NF-κB mRNA、miR-125a-5p水平与甲状腺癌术后复发的关系及NF-κB mRNA、miR-125a-5p间的相关性,采用受试者工作特征曲线评价NF-κB mRNA、miR-125a-5p检测对甲状腺癌术后复发的预测价值.结果 观察组NF-κB mRNA水平高于对照组,miR-125a-5p水平低于对照组(P<0.05).甲状腺癌患者NF-κB mRNA、miR-125a-5p水平在肿瘤大小、临床分期、血管间隙浸润及淋巴结转移方面比较差异有统计学意义(P<0.01).甲状腺癌术后复发患者肿瘤大小>5cm、临床分期Ⅲ~Ⅳ期、血管间隙浸润及淋巴结转移比例和术后 1 周NF-κB mRNA水平均高于未复发患者,术后1 周miR-125a-5p水平及NF-κB mRNA、miR-125a-5p手术前后差值绝对值均低于未复发患者(P<0.05,P<0.01).NF-κB mRNA、miR-125a-5p手术前后差值绝对值是甲状腺癌术后复发的影响因素(P<0.01).NF-κB mRNA、miR-125a-5p手术前后差值绝对值联合预测甲状腺癌术后复发的曲线下面积为0.927,高于各指标单独预测的曲线下面积(0.831、0.775)(P<0.05).相关性分析可知,甲状腺癌患者NF-κB mRNA与miR-125a-5p呈负相关(r =-0.765,P<0.01).结论 甲状腺癌患者NF-κB mRNA水平上调,miR-125a-5p水平下调,临床检测手术前后差值绝对值,可预测其术后复发情况,便于术后早期制订辅助治疗方案.
Abstract
Objective To investigate the relationship between nuclear transcription factor(NF-κB)mRNA,miR-125a-5p and clinicopathological features and prognosis in patients with thyroid cancer.Methods From May 2017 to April 2020,240 patients with thyroid cancer undergoing surgery were selected as the observation group,and 60 healthy physical ex-aminees during the same period were selected as the control group.The levels of NF-κB mRNA and miR-125a-5p were com-pared between the two groups,and the relationship between NF-κB mRNA,miR-125a-5p levels and clinicopathological char-acteristics of thyroid cancer patients was analyzed,and clinical data of patients with and without recurrence were compared.The relationship between NF-κB mRNA and miR-125a-5p levels and postoperative recurrence of thyroid cancer and the corre-lation between NF-κB mRNA and miR-125a-5p levels were compared.The predictive value of NF-κB mRNA and miR-125a-5p detection in postoperative recurrence of thyroid cancer was evaluated by receiver operating characteristic(ROC)curve.Results The expression level of NF-κB mRNA in observation group was higher than that in control group,and the level of miR-125a-5p was lower than that in control group(P<0.05).There were significant differences in NF-κB mRNA and miR-125a-5p levels in tumor size,clinical stage,vascular space invasion and lymph node metastasis in patients with thy-roid cancer(P<0.01).The tumor size>5 cm,clinical stage Ⅲ-Ⅳ,proportion of vascular space infiltration and lymph node metastasis and NF-κB mRNA level at 1 week after surgery for thyroid cancer in patients with recurrence were higher than those in patients without recurrence;the level of miR-125a-5p and the absolute difference of NF-κB mRNA and miR-125a-5p before and after surgery were lower than those without recurrence(P<0.05,P<0.01).The absolute difference of NF-κB mRNA and miR-125a-5p before and after surgery was an influential factor for postoperative recurrence of thyroid cancer pa-tients(P<0.01).The area under the ROC curve of the absolute difference of NF-κB mRNA and miR-125a-5p before and af-ter surgery in combination for prediction was 0.927,which was higher than that predicted by each index alone(0.831,0.775 respectively)(P<0.05).Correlation analysis showed that NF-κB mRNA was negatively correlated with miR-125a-5p in thy-roid cancer patients(r =-0.765,P<0.01).Conclusion NF-κB mRNA level is up-regulated and miR-125a-5p expres-sion level is down-regulated in thyroid cancer patients.Clinical detection of the absolute difference before and after surgery can predict the recurrence of thyroid cancer,which is conductive to early postoperative adjuvant therapy.
