不同内固定修复材料治疗跟骨骨折的生物相容性及修复效果研究

摘要
目的 比较不同内固定修复材料治疗跟骨骨折的生物相容性及修复效果.方法 选取2020 年5 月—2022 年5 月收治的84 例跟骨骨折为研究对象,根据使用内固定修复材料不同分为A组和B组,每组 42 例.A组选用可吸收螺钉进行内固定,B组选用金属螺钉进行内固定.观察2 组围术期指标、并发症、修复效果,以及手术前后影像学指标、红细胞免疫指标[协同肿瘤红细胞免疫花环率(ATER)、直向肿瘤红细胞花环率(DTER)、红细胞C3b受体花环率(RBC-C3bR)].结果 A组手术时间短于B组,术中出血量少于B组(P＜0.01).2 组负重时间、骨折愈合时间比较差异无统计学意义(P＞0.05).术后3 个月,2 组Gissane角、Böhler角、跟骨高度均高于术前,跟骨宽度低于术前(P＜0.05),2 组间比较差异无统计学意义(P＞0.05).术后3d,2 组ATER、RBC-C3bR、DTER均低于术前,但A组高于B组(P＜0.05).A组并发症总发生率为4.76%(2/42)低于B组的23.81%(10/42)(P＜0.05).术后3 个月,2组修复优良率比较差异无统计学意义(P＞0.05).结论 可吸收螺钉和金属螺钉在跟骨骨折内固定术中效果相当,前者在缩短手术时间、减少术中出血量及并发症、减轻机体红细胞免疫应激方面更具优势.
Abstract
Objective To compare the biocompatibility and repair effect of different internal fixation materials in the treatment of calcaneal fracture.Methods A total of 84 patients with calcaneal fracture treated from May 2020 to May 2022 were selected as the research subjects and divided into group A and group B according to different internal fixation materials,with 42 cases in each group.Absorbable screws were used for internal fixation in group A,and metal screws were used for in-ternal fixation in group B.Perioperative indexes,complications,repair effects,imaging indexes,and red cell immune indexes before and after surgery[synergistic tumor erythrocyte immune garland rate(ATER),direct tumor erythrocyte garland rate(DTER),and red blood cell C3b receptor garland rate(RBC-C3bR)]of the two groups were observed.Results The dura-tion of operation was shorter in group A than in group B,and the amount of intraoperative blood loss was less than that of group B(P＜0.01).There was no significant difference in weight-bearing time and fracture healing time between the two groups(P＞0.05).At 3 months after surgery,Gissane Angle,Bohler Angle and calcaneus height in the two groups were higher than those before surgery,and calcaneus width was smaller than that before surgery(P＜0.05),and there was no statistical signif-icance between the two groups(P＞0.05).At 3 d after operation,ATER,RBC-C3bR and DTER in the two groups were low-er than those before surgery,which,however,were higher in group A than in group B(P＜0.05).The total incidence of complications in group A was4.76%(2/42),which was lower than that of group B[23.81%(10/42)](P＜0.05).At 3 months after operation,there was no significant difference in the excellent and good rate of repair between the two groups(P＞0.05).Conclusion Absorbable screw and metal screw have similar effects in internal fixation of calcaneal fracture.The for-mer has more advantages in shortening the duration of operation,reducing intraoperative blood loss and complications,and al-leviating red blood cell immune challenge.
