摘要
目的 探索多学科协作结合阶段性干预对早产儿发育及母亲亲职压力的影响.方法 选取 2019 年 1月—2022 年5 月100 例早产儿,根据干预方案不同分为对照组48 例和观察组52 例.对照组采用常规干预,观察组采取基于多学科协作的阶段性干预疗法.观察干预前及出院后 3、6 个月后 2 组早产儿体格生长、神经行为、耳声发射(OAE)及母亲亲职压力的变化.结果 出院后3、6 个月,2 组早产儿身长、体质量、头围及精细运动、大运动、适应能力、社交行为、语言评分均较干预前升高,且观察组高于对照组(P<0.05,P<0.01).出院后 3、6 个月,2 组早产儿OAE通过率比较差异无统计学意义(P>0.05).出院后3、6 个月,2 组母亲亲职压力各维度评分均较干预前降低,且观察组低于对照组(P<0.05).结论 基于多学科协作的阶段性干预疗法可有效降低早产儿母亲亲职压力,改善早产儿体格、神经、心理发育及认知功能,利于提高早产儿生长质量.
Abstract
Objective To explore the effects of multi-disciplinary collaboration combined with phased intervention on the development and maternal parenting stress of premature infants.Methods In total,100 preterm infants from January 2019 to May 2022 were selected and divided into the control group(n =48)and the observation group(n =52)according to different intervention programs.The control group received conventional intervention,and the observation group received phased intervention therapy based on multidisciplinary collaboration.The changes of physical growth,neurobehavior,otoa-coustic emission(OAE)of preterm infants and maternal parenting stress were observed before intervention and at 3 and 6 months after discharge.Results At 3 and 6 months after discharge,body length,body weight,head circumference,and the scores of fine motor,gross motor,adaptive ability,social behavior and language of the two groups were higher than those be-fore intervention,and higher in the observation group than in the control group(P<0.05,P<0.01).There was no signifi-cant difference in the passing rate of OAE between the observation group and the control group at 3 and 6 months after dis-charge(P>0.05).At 3 and 6 months after discharge,the scores of all dimensions of maternal parenting stress in the two groups were lower than those before intervention,and lower in the observation group than in the control group(P<0.05).Conclusion The phased intervention therapy based on multidisciplinary collaboration can effectively reduce the maternal par-enting stress of preterm infants,improve the physical development,neuropsychological development and cognitive function of preterm infants,and improve the growth quality of preterm infants.
