摘要

目的 探究超微血管显像(SMI)技术联合甲状腺影像学报告及数据系统(TI-RADS)对甲状腺结节良恶性的鉴别诊断价值.方法 选取2020 年1 月—2023 年1 月就诊的甲状腺结节110 例,均行常规超声、SMI检查,依据常规超声行TI-RADS分级.以手术病理结果为金标准,对比SMI、TI-RADS单一及二者联合检查对甲状腺结节良恶性的诊断效能,且通过SMI对结节进行血流分级.结果 110 例经手术病理证实甲状腺恶性结节(甲状腺癌)58 例,良性结节52 例.以病理结果为金标准,SMI诊断甲状腺恶性结节的敏感度、特异度、准确度分别为 91.38%、80.77%、86.36%,TI-RADS分别为75.86%、76.92%、76.36%,SMI联合TI-RADS分别为94.83%、88.46%、91.82%.SMI、SMI联合TI-RADS诊断甲状腺恶性结节的敏感度、准确度均高于TI-RADS(P<0.05).SMI模式下,甲状腺恶性结节血流分级2~3 级比例高于良性结节(P<0.01).结论 SMI联合TI-RADS能有效提高甲状腺恶性结节诊断效能,SMI血流分级有利于甲状腺结节定性诊断.

Abstract

Objective To explore the value of superb microvascular imaging(SMI)combined with thyroid imaging report and data system(TI-RADS)in differential diagnosis of benign and malignant thyroid nodules.Methods A total of 110 patients with thyroid nodules treated from January 2020 to January 2023 were selected,and underwent routine ultrasound and SMI examination,and TI-RADS classification was performed according to routine ultrasound.Using surgical and patholog-ical results as the gold standard,the diagnostic efficiency of SMI and TI-RADS alone and in combination for benign and malig-nant thyroid nodules was compared,and blood flow classification of nodules was performed by SMI.Results Among 110 ca-ses,58 were malignant thyroid nodules(thyroid cancer)and 52 were benign.Using pathological results as the gold standard,the sensitivity,specificity and accuracy of SMI in the diagnosis of malignant thyroid nodules were 91.38%,80.77%and 86.36%,respectively,those of TI-RADS were75.86%,76.92%and 76.36%,respectively,and those of SMI combined with TI-RADS were 94.83%,88.46%and 91.82%,respectively.The sensitivity and accuracy of combination of SMI,SMI and TI-RADS in the diagnosis of malignant thyroid nodules were higher than those of TI-RADS(P<0.05).In SMI mode,the proportion of malignant thyroid nodules in blood flow grade 2-3 was higher than that of benign nodules(P<0.01).Conclu-sion SMI combined with TI-RADS can effectively improve the diagnostic efficiency of malignant thyroid nodules,and SMI blood flow grading is conductive to the qualitative diagnosis of thyroid nodules.
