摘要

目的 分析支气管哮喘(BA)急性发作患者治疗前后血清血栓素 B2(TXB2)、CC 趋化因子受体 7(CCR7)水平变化及与肺功能的关系.方法 选取 2019 年 5 月—2022 年 5 月收治的 203 例急性发作BA.比较BA急性发作患者治疗前后血清TXB2、CCR7 水平及肺功能情况.依据治疗后肺功能损伤程度,将其分为中重度损伤组和轻度损伤组.分析影响BA急性发作患者肺功能损伤加重的危险因素,并探究血清TXB2、CCR7 与肺功能的相关性.结果 治疗后BA急性发作患者血清TXB2、CCR7 水平较治疗前下降,用力肺活量(FVC)、第 1 秒用力呼气容积(FEV1)、最大呼吸流量(PEF)及 FEV1/FVC 较治疗前有所上升(P<0.05).多因素 Logistic 回归分析显示,血清TXB2、CCR7 水平过表达是影响BA急性发作患者肺功能损伤加重的独立危险因素(P<0.05).BA急性发作患者中,肺功能中、重度损伤 31 例,肺功能轻度损伤 172 例.中重度损伤组血清 TXB2、CCR7 水平高于轻度损伤组(P<0.01).BA急性发作患者血清TXB2、CCR7 水平分别与FVC、FEV1、PEF及FEV1/FVC呈负相关(P<0.01).结论 治疗后BA急性发作患者血清TXB2、CCR7 水平显著下降,二者是影响患者肺功能损伤加重的危险因素,密切监测血清TXB2、CCR7 水平变化可为临床治疗提供指导.

Abstract

Objective To analyze the relationship between changes in serum thromboxane B2(TXB2),CC chemo-kine receptor 7(CCR7)levels with pulmonary function in patients with acute attack of bronchial asthma(BA)before and af-ter treatment.Methods A total of 203 patients with acute attack of BA admitted from May 2019 to May 2022 were selected.The levels of serum TXB2,CCR7 and pulmonary function of patients with acute attack of BA were compared before and after treatment.According to the degree of pulmonary function injury after treatment,the patients were divided into moderate and severe injury group and mild injury group.The risk factors affecting the exacerbation of pulmonary function injury in patients with acute attack of serum BA were analyzed,and the correlation between serum TXB2,CCR7 and pulmonary function was ex-plored.Results After treatment,the levels of TXB2 and CCR7 in patients with BA acute attack were decreased compared with those before treatment,and the forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),peak expiratory flow(PEF)and FEV1/FVC were increased compared with those before treatment(P<0.05).Multivariate Logis-tic regression analysis showed that overexpression of serum TXB2 and CCR7 levels was an independent risk factor for the exac-erbation of pulmonary function injury in BA patients(P<0.05).Among the patients with acute attack of BA,31 cases had moderate or severe impairment of pulmonary function,and 172 cases had mild impairment of pulmonary function.The levels of serum TXB2 and CCR7 in moderate and severe injury group were higher than those in mild injury group(P<0.01).The lev-els of serum TXB2 and CCR7 in patients with acute attack of BA were negatively correlated with FVC,FEV1,PEF and FEV1/FVC,respectively(P<0.01).Conclusion After treatment,the levels of serum TXB2 and CCR7 decreased significantly in patients with acute BA attack,which are risk factors for the exacerbation of pulmonary function injury in patients.Close moni-toring of the changes of serum TXB2 and CCR7 levels can provide guidance for clinical treatment.
