益气化痰祛瘀方对鼻中隔偏曲矫正术后咽鼓管功能和并发症的影响

摘要
目的 探讨益气化痰祛瘀方对鼻中隔偏曲矫正术后咽鼓管功能和并发症的影响.方法 选择2019 年6 月—2022 年6 月行鼻中隔偏曲矫正术的患者110 例,采用随机数字表法分为观察组和对照组,每组55 例.2 组均行鼻中隔偏曲矫正手术,术后给予常规治疗,观察组在此基础上给予益气化痰祛瘀方.采用七项咽鼓管功能障碍症状评分量表(ETDQ-7)评估2 组手术前后咽鼓管功能;采用鼻阻力仪和糖精试验法分别检测2 组手术前后鼻阻力值和鼻腔黏膜纤毛输送率,评价鼻腔黏膜损伤情况;记录2 组术后中耳炎发生情况;采用分光光度计法测定 2 组手术前后咽鼓管灌洗液表面活性物质(ETS)磷脂含量.结果 2 组术后ETDQ-7 评分低于治疗前,且观察组低于对照组(P＜0.01).2 组术后鼻阻力值和鼻腔黏膜纤毛输送率低于术前;观察组鼻阻力值低于对照组,鼻腔黏膜纤毛输送率高于对照组(P＜0.01).观察组术后中耳炎发生率3.64%(2/55)低于对照组18.18%(10/55)(P＜0.05).2 组术后ETS磷脂含量高于术前,且观察组高于对照组(P＜0.01).结论 益气化痰祛瘀方可促进鼻中隔偏曲矫正术后患者咽鼓管功能的恢复,有助于鼻腔黏膜修复,并发症少.
Abstract
Objective To explore the effect of Yiqi Huatan Quyu recipe(therapy of supplementing qi,resolving phlegm and removing blood stasis)on eustachian tube function in patients undergoing correction of deviation of nasal septum(DNS).Methods A total of 110 patients who underwent correction of DNS from June 2019 to June 2022 were selected and divided into observation group(n =55)and control group(n =55)by random number table method.The two groups were treated with surgery for correction of DNS,and the observation group was treated with Yiqi Huatan Quyu recipe.The eustachi-an tube function of the two groups was evaluated by the seven-item Eustachian tube Dysfunction Symptom Scale(ETDQ-7)be-fore and after operation.Nasal resistance and nasal mucociliary delivery rate before and after operation were detected by nasal resistance meter and saccharin test,respectively,to evaluate the nasal mucosal injury.The occurrence of otitis media after op-eration was recorded in the two groups.The content of eustachian tube surfactant(ETS)phospholipid in irrigation fluid of eu-stachian tube before and after operation was determined by spectrophotometer.Results The ETDQ-7 score of the two groups was lower than that before treatment,and lower in the observation group than in the control group(P＜0.01).The nasal re-sistance value and the delivery rate of nasal mucocilia in both groups were lower than those before operation.The nasal resist-ance value of observation group was lower than that of control group,and the delivery rate of nasal mucocilia was higher than that of control group(P＜0.01).The incidence of postoperative otitis media in the observation group was 3.64%(2/55),which was lower than that in the control group[18.18%(10/55)](P＜0.05).The content of ETS phospholipid in irrigation fluid of pharyngodryngeal tube in the two groups was higher than that before treatment,and higher in observation group than in control group(P＜0.01).Conclusion Yiqi Huatan Quyu recipe can promote the recovery of eustachian tube function in pa-tients undergoing correction of DNS,contribute to the repair of nasal mucosa,and produce fewer complications.
