摘要
目的 探究三白益肾方联合治疗脾肾亏虚型慢性肾炎的临床效果,分析其对中医症候积分、肾功能的影响.方法 选取2020 年1 月—2022 年1 月收治的脾肾亏虚型慢性肾炎110 例为研究对象,随机分为对照组和观察组,各55 例.对照组予以基础治疗配合常规西药治疗,观察组在此基础上予以三白益肾方治疗.统计2 组临床疗效、中医症候疗效、不良反应发生率及治疗前、治疗1 个月、治疗2 个月24h尿蛋白定量、中医症候积分;对比2 组治疗前后肾功能指标[血肌酐(Scr)、尿微量白蛋白肌酐比(UACR)、尿素(BUN)、尿红细胞计数]、微炎症状态、免疫功能(CD3 +、CD4 +、CD4 +/CD8 +)水平.结果 观察组临床疗效、中医症候疗效优于对照组(P<0.05,P<0.01).治疗1 个月、2 个月后,观察组24h尿蛋白定量、中医症候积分低于对照组(P<0.01);2 组治疗1 个月、2 个月Scr、UACR、BUN及尿红细胞计数比较差异无统计学意义(P>0.05);治疗1 个月、2 个月后,观察组血清肿瘤坏死因子-α、超敏C反应蛋白、白细胞介素-6 水平低于对照组,CD3 +、CD4 +及CD4 +/CD8 +高于对照组(P<0.01).2 组不良反应发生率比较差异无统计学意义(P>0.05).结论 三白益肾方联合治疗脾肾亏虚型慢性肾炎疗效确切,可改善临床症状、肾功能,抑制炎症反应,提高机体免疫力水平,且具有一定安全性.
Abstract
Objective To investigate the clinical effect of Sanbai Yishen Formula in the combined treatment of chron-ic nephritis of spleen-kidney deficiency type,and to analyze its effect on traditional Chinese medicine(TCM)syndrome score and renal function.Methods A total of 110 patients with chronic nephritis of spleen and kidney deficiency type admitted from January 2020 to January 2022 were selected as the research subjects and randomly divided into a control group and an ob-servation group,with 55 cases in each group.The control group received basic treatment combined with conventional Western medicine treatment,while the observation group received Sanbai Yishen formula on this basis.The clinical efficacy,efficacy of TCM syndrome,incidence of adverse reactions,and 24-hour urine protein quantitation,TCM syndrome scores before treat-ment,at 1 month and 2 months after treatment were statistically analyzed in both groups.The renal function indicators[serum creatinine(Scr),urinary albumin creatinine ratio(UACR),blood urea nitrogen(BUN),urinary red blood cell count],mi-croinflammatory state,and immune function(CD3 +,CD4 +,CD4 +/CD8 +)levels before and after treatment were com-pared between the two groups.Results The clinical efficacy and efficacy of TCM syndrome in the observation group were su-perior to those in the control group(P<0.05,P<0.01).At 1 month and 2 months after treatment,the24-hour urine pro-tein quantitative and TCM syndrome scores in the observation group were lower than those in the control group(P<0.01);there was no significant difference in Scr,UACR,BUN,and urine red blood cell count between the two groups at 1 month and 2 months after treatment(P>0.05).At1 month and2 months after treatment,the serum tumor necrosis factor-α,hypersen-sitive C-reactive protein,and interleukin-6 levels in the observation group were lower than those in the control group,while CD3 +,CD4 +,and CD4 +/CD8 + were higher than those in the control group(P<0.01).There was no significant differ-ence in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Sanbai Yishen formula is effec-tive in the treatment of chronic nephritis of spleen-kidney deficiency type,which can improve clinical symptoms and renal function,inhibit inflammatory reaction,improve the level of body immunity,and has a certain degree of safety.
