Correlation of IL-6,TNF-α,and MCP-1 in Gingival Crevicular Fluid with CAT Grading and Their Predictive Value for Chronic Periapical Periodon-titis in Preschool Children with Caries
Objective To investigate the correlation between interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),monocyte chemotactic protein-1(MCP-1)and caries activity test(CAT)grading in gingival crevicular fluid of preschool children with caries,and the predictive value of chronic periapical periodontitis.Methods A total of 480 children aged 3-6 years receiving oral examination from January 2020 to June 2021 were selected and divided into occurrence group and non-occurrence group according to the incidence of dental caries.The levels of IL-6,TNF-α and MCP-1 in gingival creval flu-id were compared between the two groups and between patients with different levels of caries risk.The levels of IL-6,TNF-α and MCP-1 in gingival crevicular fluid were compared between children with chronic periapical inflammation(chronic periapi-cal inflammation subgroup)and children without chronic periapical inflammation(non-chronic periapical inflammation sub-group)at admission,and the correlation between IL-6,TNF-α and MCP-1 in gingival crevicular fluid and caries risk was ana-lyzed by Spearman correlation analysis.Receiver operating characteristic(ROC)curve was used to analyze the value of IL-6,TNF-α,MCP-1 and CAT grading for predicting chronic periapical inflammation.Results IL-6,TNF-α and MCP-1 in gingi-val crevicular fluid were higher in occurrence group than in non-occurrence group(P<0.01).The levels of IL-6,TNF-α and MCP-1 in gingival crevicular fluid increased with the increase of caries risk(P<0.05).IL-6,TNF-α and MCP-1 in gingival crevicular fluid were positively correlated with caries risk(P<0.01).The levels of IL-6,TNF-α and MCP-1 in gingival crev-icular fluid in chronic periapical inflammation subgroup were higher than those in non-chronic periapical inflammation sub-group(P<0.01).The ROC curves of IL-6,TNF-α,MCP-1 and CAT grading in gingival crevicular fluid for predicting chro-nic periapical inflammation showed that the area under the curve(AUC)was 0.732,0.774,0.743 and 0.745,respectively,in which TNF-α was the most sensitive and CAT grading was the most specific in prediction.The AUC,sensitivity and speci-ficity of IL-6,TNF-α and MCP-1 combined with CAT grading in predicting chronic periapical inflammation were 0.924,0.919 and 0.806,respectively,which were higher than those predicted by each index alone.Conclusion IL-6,TNF-α and MCP-1 in gingival crevicular fluid are closely related to caries activity and the occurrence of chronic periapical periodontitis in preschool children with caries.The combination of the three factors with CAT grading may be an effective method for predic-ting chronic periapical periodontitis in preschool children with caries,and can provide a reference for clinical prevention and treatment of caries.
