Objective To summarize the characteristics of atypical primary angle-closure glaucoma(PACG)and to discuss the causes of misdiagnosis and preventive measures.Methods The clinical data of 5 patients with atypical PACG treated from August 2020 to October 2022 were retrospectively analyzed.Results Of the 5 patients,3 presented with monoc-ular visual acuity decrease without obvious cause,1 with headache and eye distension,and 1 with nausea,vomiting and slight orbital pain.Three patients were misdiagnosed as optic neuritis,1 as migraine,and 1 as acute gastroenteritis.Misdiagnosis lasted from 4 to 15 d.Relevant examinations[intraocular pressure(IOP),visual field,slit-lamp microscopy,anterior cham-ber angle by microscopy,optical coherence tomography(OCT)of optic disc]were completed,and combined with the patient's symptoms,atypical PACG was finally diagnosed.Four patients underwent trabeculectomy,and the prognosis was good.One patient declined surgery and had another acute attack during follow-up.Conclusion Atypical PACG has various clinical man-ifestations and no typical symptoms,which is easily confused with other medical diseases,thus leading to misdiagnosis.It is necessary to strengthen the ability of differential diagnosis of atypical PACG and enhance the vigilance of this disease,thus re-ducing the misdiagnosis.
