Objective To investigate the serum levels of homocysteine(Hcy),25-hydroxyvitamin D3[25(OH)D3],interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and their correlation in patients with type 2 diabetes mellitus(T2DM)complicated with sarcopenia.Methods Thirty T2DM patients complicated with sarcopenia treated from January to December 2023 were selected as the sarcopenia group,and another 30 T2DM patients treated during the same period were se-lected as the non-sarcopenia group.The clinical data were collected at admission,and the walking speed,grip strength,the Simple Five item Scoring Scale for Sarcopenia(SARC-F)scores and serum levels of Hcy,25(OH)D3,IL-6 and TNF-α were compared between the sarcopenia group and the non-sarcopenia group.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum Hcy,25(OH)D3,IL-6 and TNF-α in patients with T2DM complicated with sarcopenia,and the correlation between these indexes and walking speed,grip strength and SARC-F score in patients with T2DM complicated with sarcopenia was analyzed.Results The walking speed,grip strength and serum 25(OH)D3 level in the sarcopenia group were lower than those in the non-sarcopenia group,while the SARC-F score and serum Hcy,IL-6 and TNF-α levels in the sarcopenia group were higher than those in the non-sarcopenia group(P<0.05,P<0.01).ROC curve analysis showed that serum Hcy,25(OH)D3,IL-6 and TNF-α alone and in combination had better value in the diagnosis of T2DM complicated with sarcopenia,and the combined detection had the optimal value.Serum Hcy,IL-6 and TNF-α in pa-tients with T2DM complicated with sarcopenia were negatively correlated with walking speed and grip strength(P<0.01),and positively correlated with SARC-F score(P<0.01).25(OH)D3 was positively correlated with walking speed and grip strength(P<0.01),and negatively correlated with SARC-F score(P<0.01).Conclusion The levels of serum Hcy,IL-6 and TNF-α were significantly increased in patients with T2DM complicated with sarcopenia,and the levels of 25(OH)D3 were significantly decreased,which was correlated with the occurrence of sarcopenia.
